もって落札価格とするので、入札者は、消費
税に係る課税事業者であるか免税事業者であ
るかを問わず、見積もった契約金額の110分
の100に相当する金額を入札書に記載するこ

2競争参加資格
(1)予算決算及び会計令第70条の規定に該当し
ない者であること。なお、未成年者、被保佐
人又は被補助人であっても、契約締結のため
に必要な同意を得ている者は、同条中、特別
の理由がある場合に該当する。
(2)予算決算及び会計令第71条の規定に該当し
ない者であること。
(3)令和07・08・09年度厚生労働省競争参加資
格(全省庁統一資格)において、近畿地域に
おける資格を有しており「物品の販売」、「役
務の提供等」の「A」、「B」又は「C」等級
に格付けされる者であること。
(4)厚生労働省から取引停止の措置を受けてい
る期間中の者でないこと。
(5)資格審査申請書又はその添付書類に虚偽の
事実を記載した者でないこと。
(6)経営の状況又は信用度が極度に悪化してい
る者でないこと。
(7)厚生年金保険、健康保険、船員保険、国民
年金、労働者災害補償保険及び雇用保険が適
用される者にあっては、この入札の入札書提
出期限の直近2年間(労働者災害補償保険及
び雇用保険については2保険年度)の保険料
の滞納がない者であること。
※労働者災害補償保険及び雇用保険について
は、当該年度における年度更新手続を完了す
べき日が未到来の場合にあっては前年度及び
前々年度、年度更新手続を完了すべき日以降
の場合にあっては当該年度及び前年度の保険
料について滞納がない(分納が認められてい
るものについては納付期限が到来しているも
のに限る。)こと。
(8)この入札の入札書提出期限の直近1年間に
おいて、厚生労働省が所管する法令に違反し
たことにより送検され、行政処分を受け、又
は行政指導(行政機関から公表されたものに
限る。)を受けた者にあっては、本件業務の公
正な実施又は本件業務に対する国民の信頼の
確保に支障を及ぼすおそれがないこと。
(9)労働関係法令を遵守している者であるこ
と。

4電子調達システムの利用
本件は、電子調達システムを利用した応札及
び入札手続きにより実施するものとする。なお、
電子調達システムにより難い場合は、「紙入札方
式による参加理由書」により紙入札方式である
旨を申し出た場合に限り紙入札方式に変えるこ
とができる。
5その他
(1)入札及び契約手続において使用する言語及
び通貨日本語及び日本国通貨。
(2)入札保証金及び契約保証金免除。
(3)入札者に求められる義務この一般競争に
参加を希望する者は、電子調達システムにお
ける所定の手続による入札手続を行うか、又
は封印した入札書に入札説明書に示す書類を
添付して入札書提出期限日までに提出しなけ
ればならない。

(4)入札の無効本公告に示した競争参加資格
のない者の提出した入札書、 入札者に求めら
れる義務を履行しなかった者の提出した入札
書、関係書類に虚偽の記載をした者の提出し
た入札書及び入札に関する条件に違反した者
の提出した入札書は無効とする。
(5)契約書作成の要否要。
(6)落札者の決定方法予算決算及び会計令第
79条の規定に基づいて作成された予定価格の
制限の範囲内で最低価格をもって有効な入札
を行った入札者を落札者とする。

false statement in the application for qua-

tached thereto.
⑥ Not be a person whose business condi-
tions or creditworthiness has deteriorat-
ed significantly.
⑦Where the Employees' Pension Insur-
ance, Health Insurance, Seamen's Insur-
ance, National Pension, Acci-
dent Compensation Insurance, and Em-
ployment Insurance are applicable, the
bidder must have no delinquency in the
payment of insurance premiums for the
two years immediately preceding the de-
adline for submission of bids under this
tender (or, in the case of Workers' Acci-
dent Compensation Insurance and Em-
ployment Insurance, for the most recent
two insurance fiscal years). For Workers'
Accident Compensation Insurance and
Employment Insurance, if the due date
for completion of the annual renewal pro-
cedures for the relevant fiscal year has
not yet arrived, there must be no de-
linquency in the payment of insurance
premiums for the previous and second
previous fiscal years. If the due date has
already passed, there must be no de-
linquency in the payment of insurance
premiums for the relevant fiscal year and
the previous fiscal year. (Where install-
ment payment has been approved, this re-
quirement applies only to premiums for
which the payment due date has already
arrived.)
⑧In the case of a person who, within one
year prior to the deadline for submission
of tenders for this procurement, has been
referred for prosecution, subjected to an
administrative disposition, or received
administrative guidance (limited to those
publicly announced by an administrative
authority) as a result of violating laws
and regulations administered by the Min-
istry of Health, Labour and Welfare, such
person shall not be deemed likely to
impair the fair execution of the subject
services or the maintenance of public co-
nfidence in such services
