て、「使用薬剤の薬価(薬価基準)に収載され

(5)入札方法
①1(2)で示す医薬品についてそれぞれ入札
に付する。
②入札金額については、納入に要する一切
の費用を織り込んだ上で医薬品ごとにそれ
ぞれの医薬品目の単価を記載すること。
③入札単価については、消費税に係る課税
事業者であるか免税事業者であるかを問わ
ず、見積もった契約金額の本体価格(消費
税法第28条第1項及び第29条並びに地方税
法第72条の82及び第72条の83の規定に基づ
き契約金額に110分の100を乗じて得た金額
をいう。なお、税法の改定により消費税等
の税率が変更された場合には、当該改正税
法施行日以降における上記本体価格は変動
後の消費税等の税率により計算した額とす
る。)を記載すること。
(6)その他独立行政法人国立病院機構契約事
務取扱細則(以下、「契約細則」という。)第22
条の規定に基づき単価契約とする。
2競争参加資格
(1)契約細則第5条の規定に該当しない者であ
ること。なお、未成年者、被保佐人又は被補
助人であっても、契約締結のために必要な同
意を得ているものは、同条中、特別の理由が
ある場合に該当する。
(2)契約細則第6条の規定に該当しない者であ
ること。
(3)厚生労働省競争参加資格(全省庁統一資格)
「物品の購入等」のA、B又はCの等級に格
付され、北海道地域の競争参加資格を有する
者であること。なお、当該競争参加資格につ
いては、令和8年3月31日付け号外政府調達
第57号の官報の競争参加者の資格に関する公
示の別表に掲げる申請受付窓口において随時
受け付けている。
(4)医薬品、医療機器等の品質、有効性及び安
全性の確保等に関する法律に基づいて医薬品
の一般販売業の許可を受けていることを証明
した者であること。
(5)購入される医薬品を経理責任者が指定する
日時、場所に十分に納品することができるこ
とを証明した者であること。
(6)契約細則第4条の規定に基づき、経理責任
者が定める資格を有する者であること。

について説明を求められた場合はそれに応じ
なければならない。
(4)入札の無効本公告に示した競争参加資格
のない者の提出した入札書、入札者に求めら
れる義務を履行しなかった者の提出した入札
書は無効とする。

(6)落札者の決定方法契約細則第21条の規定
に基づいて作成された予定価格の制限の範囲
内で最低価格をもって有効な入札を行った入

tion concerning Contract for NHO;
③ have Grade A, B or C in "Sales of prod
