次のとおり一般競争入札に付します。

う什器購入及び設置」

(6)入札方法入札金額は総価で行う。なお、
落札決定に当たっては、入札書に記載された
金額に当該金額の10パーセントに相当する額
を加算した金額(当該金額に1円未満の端数
があるときは、その端数金額を切り捨てるも
のとする。)をもって落札価格とするので、入
札者は、消費税に係る課税事業者であるか免
税事業者であるかを問わず、見積もった契約
金額の110分の100に相当する金額を入札書に
記載すること。
2競争参加資格
(1)予算決算及び会計令第70条の規定に該当し
ない者であること。なお、未成年者、被保佐
人又は被補助人であっても、契約締結のため
に必要な同意を得ている者は、同条中、特別
の理由がある場合に該当する。
(2)予算決算及び会計令第71条の規定に該当し
ない者であること。
(3)令和7・8・9年度厚生労働省競争参加資
格(全省庁統一資格)において、物品の販売
及び役務の提供の「A」、「B」又は「C」等
級に格付けされ、競争参加資格を有する者で
あること。
(4)次の各号に掲げる制度が適用される者に
あっては、この入札の入札書提出期限の直近
2年間に次の(⑤及び⑥については2保険年
度)保険料について滞納がないこと,
①厚生年金保険②健康保険(全国健康保険
協会が管掌するもの)③船員保険④国民
年金⑤労働者災害補償保険⑥雇用保険
(5)資格審査申請書及びその添付書類に虚偽の
事実を記載していないと認められる者である

(6)経営の状況又は信用度が極度に悪化してい
ないと認められる者であること。
(7)商法その他の法令の規定に違反して営業を
行った者でないこと。
(8)厚生労働省から指名停止の措置を受けてい
る期間中の者でないこと。
(9)過去1年以内に、厚生労働省所管法令違反
により行政処分等を受けていないこと。

(2)入札説明書の交付期間及び競争参加資格等
確認関係書類の提出期間令和8年7月13日
8時30分から令和8年9月1日16時まで競争
参加資格等確認関係書類を上記期間中に、上
記3(1)宛てに提出すること。
※電子メールにて入札説明書の交付を希望す
る者は、上記3(1)担当者へ連絡を入れるこ
と。

(4)入札書の提出方法入札書の提出にあたっ
ては、原則、政府電子調達システムで行う。
なお、政府電子調達システムによりがたい者
は、支出負担行為担当官に書面により申し出
た場合に限り紙入札方式に変えることができ
る。紙入札方式を希望する場合、上記3(1)へ
上記3(2)の競争参加資格等確認関係書類の提
出期間中に、「電子入札案件の紙入札方式での
参加について」を提出すること。

(1)契約手続において使用する言語及び通貨
日本語及び日本国通貨
(2)入札保証金及び契約保証金免除
(3)入札者に要求される事項この一般競争に
参加を希望する者は、競争参加資格等確認関
係書類を令和8年9月1日16時までに提出し
なければならない。入札者は、契約担当官等
から当該書類に関し説明を求められた場合
は、これに応じなければならない。応札にい
たるまでの諸経費は、応札希望者の負担とす
る。仮に不落札又は事前に提出すべき確認書
類により応札できなかった場合も同様とす
る。
(4)入札の無効
①本公告に示した競争参加資格のない者、
入札条件に違反した者又は入札者に求めら
れる義務を履行しなかった者の提出した入
札書は無効とする。
②入札書を受領した場合であって、当該競
争参加資格審査が開札日時までに終了しな
いとき又は資格を有すると認められなかっ
たときは、当該入札書は無効とする。
③支出負担行為担当官が別に指定する暴力
団等に該当しない旨の誓約書を提出せず、
又は虚偽の誓約をし、若しくは誓約書に反
することとなったときは、当該者の入札を
無効とする。
④支出負担行為担当官が別に指定する保険
料納付に係る申立書を提出せず、又は虚偽
の申立をし、社会保険料及び労働保険料等
に滞納が認められ、申立書に反することと
なったときは、当該者の入札を無効とする。
(5)担当者等から提出される契約関係書類につ
いては、事業者としての決定であること。ま
た、押印が省略された契約関係書類に虚偽記
載等の不正が発覚した場合は、契約解除や違
約金を徴取する場合がある。
(6)契約書作成の要否要
原則、契約書の締結は電子契約によること。
ただし、電子契約により難い者は、紙による
契約書作成を認める。
(7)落札者の決定方法本公告に示した業務を
履行できると支出負担行為担当官が判断した
入札者であって、予算決算及び会計令第79条
の規定に基づいて作成された予定価格の制限
の範囲内で最低価格をもって有効な入札を
行った者を落札者とする。

(8)手続きにおける交渉の有無無
(9)契約締結日等契約締結日(履行期間又は
契約期間の初日)までに政府予算(暫定予算
案を含む。)が成立していない場合は、契約締
結日は予算が成立した日以降とする。また、
暫定予算となった場合は、契約内容等につい
て変更が生じる可能性や本案件に係る予算の
決定状況によって仕様の内容について変更が
生じる可能性があるので、その際は双方で別
途協議する。

(7)Bid method: The bid price is the total
price Do. In addition, In determining a suc-
cessful bid, an amount obtained by adding
an amount corresponding to 10% of the
amount to the amount described in the bid
book (if the amount has a fraction of less
than 1 yen, the fractional amount shall be
rounded down) Therefore, the bidder shall
indicate the amount equivalent to 100/110
of the estimated contract amount regard-

(8)Participation competition qualification:
①Not come under Article70 of the Cabi-
net Order concerning the Budget, Audit-
ing and Accounting. Furthermore, mi-
nors, Person under Conservatorship or
Person under Assistance that obtained
the consent necessary for concluding a
contract may be applicable under case of
special reasons within the said clause.
②Not come under Article 71 of the Cabi-
net Order concerning the Budget, Audit-
ing and Accounting.
③Have Grade A, B or C on "sale of prod-
uct" and "Provision of Services", in terms
of the qualification for participating in
tenders by the Ministry of Health, La-
bour and Welfare (Single qualification
for every ministry and agency) in the fis-
cal years 2025, 2026 and 2027.
④Join social insurance (welfare annuity
insurance, health insurance, seamen's in-
surance, national pension, worker's com-
pensation insurance, unemployment in-
surance) and pay the social insurance pre-
miums and have no Delinquency in the
past 2 years.
⑤ A Person who is deemed not to de-
scribe any false facts in the request for
examination of qualification and its ac-
companying documents.
⑥ A person whose business situation or
trustworthiness is deemed not to have
significantly deteriorated and whose
proper performance of a contract can be
guaranteed.
⑦The applicant must not have conducted
business in violation of the provisions of
the Commercial Code or other laws and
regulations.
⑧ The applicant must not be under the
period of suspension of nomination by the
Ministry of Health, Labour and Welfare.
⑨The applicant must not have received
any administrative punishment within the
past year for violating the laws and regu-
lations under the jurisdiction of the Min-
istry of Health, Labour and Welfare
