The Impact of Preoperative Statin Therapy on Clinical Outcomes in Patients Undergoing Coronary Artery Bypass Grafting: A Meta-Analysis of More Than 170,000 Patients

BACKGROUND
Preoperative statin therapy has been associated with improved outcomes after coronary artery bypass grafting (CABG). However, data are conflicting and no meta-analysis has been performed to date.
METHODS
We searched PubMed, EMBASE, Cochrane Library, ISI Web of Science, Scopus, BioMed Central and clinical trial registries for randomized controlled trials (RCTs) and observational studies comparing preoperative statin therapy versus no statin therapy in patients undergoing CABG.
RESULTS
A total of 17 studies (3 RCTs and 14 observational studies) involving 170,669 patients were included. Preoperative statin therapy was associated with a significant reduction in all-cause mortality (OR 0.66, 95% CI 0.57-0.77, p<0.001), myocardial infarction (OR 0.69, 95% CI 0.58-0.82, p<0.001), atrial fibrillation (OR 0.64, 95% CI 0.54-0.76, p<0.001), stroke (OR 0.72, 95% CI 0.59-0.88, p=0.001), and acute kidney injury (OR 0.68, 95% CI 0.55-0.84, p<0.001).
CONCLUSION
Preoperative statin therapy is associated with improved short-term outcomes in patients undergoing CABG. These findings support the use of statins before CABG surgery.
