次のとおり一般競争入札に付します。

(6)入札方法入札金額については、別紙仕様
書に規定するもの等履行に要する一切の費用
を含めた額とすること。
なお、落札者決定に当たっては、入札書に
記載された金額に当該金額の10パーセントに
相当する額を加算した金額(当該金額に1円
未満の端数があるときは、その端数金額を切
り捨てるものとする。)をもって落札価格とす
るので、入札者は、消費税に係る課税事業者
であるか免税事業者であるかを問わず、見積
もった契約金額の110分の100に相当する金額
を入札書に記載すること。
2入札参加資格
(1)独立行政法人国立病院機構契約事務取扱細
則(以下、契約細則という。)第5条の規定に
該当しない者であること。なお、未成年者、
被保佐人又は被補助人であっても、契約締結
のために必要な同意を得ているものは、同条
中、特別の理由がある場合に該当する。
(2)契約細則第6条の規定に該当しない者であ
ること。
(3)厚生労働省競争参加資格(全省庁統一資格
令和7・8・9年度)の「役務の提供等(賃
貸借)」において、A、B又はC等級に格付け
され近畿地区の競争資格を有するものである
こと。なお、当該競争参加資格については、
令和7年3月31日付け号外政府調達第57号の
官報の競争参加者の資格に関する公示の別表
に掲げる申請受付窓口において随時受け付け
ている。
(4)契約細則第4条の規定に基づき、経理責任
者が定める資格を有する者であること。

参加を希望する者は、封印した入札書に本公
告に示した内容を履行できることを証明する

類に関し説明を求められた場合は、それに応
じなければならない。
(4)入札の無効公告に示した競争参加資格の
ない者が提出した入札書、入札者に求められ
る義務を履行しなかった者の提出した入札書
は無効とする。

(6)落札者の決定方法契約細則第21条の規定
に基づいて作成された予定価格の制限の範囲
内であり、かつ、経理責任者が入札説明書等
で指定する性能、機能等の要件のうち、必須
とした項目にかかる基準を全て満たしている
提案をしたものの中から、経理責任者が入札
説明書で定める総合評価の方法をもって落札
者を定める。
