在中」と朱記しなければならない。再度入札
となることも考慮し、必要に応じて複数枚の
入札書を提出すること(封筒に、必要事項の
他、何回目の入札書であるかを必ず明記す
る。)。

応札者は立ち会わないこととし、入札結果
については応札者全員にメール等で連絡す
る。
5その他
(1)契約手続きにおいて使用する言語及び通貨
日本語及び日本国通貨に限る。
(2)入札保証金及び契約保証金免除
(3)入札者に要求される事項この入札に参加
を希望する者は、上記2の競争参加資格を有
することを証明する書類を令和8年3月6日
10時00分までに提出しなければならない。
また、入札に参加を希望する者は、上記証
明書類と合わせて暴力団等に該当しない旨の
誓約書を提出しなければならない。
入札者は、支出負担行為担当官から当該書
類に関し説明を求められた場合は、これに応
じなければならない。
(4)入札の無効本公告に示した競争参加資格
のない者の提出した入札書、入札者に求めら
れる義務を履行しなかった者の提出した入札
書は無効とする。(3)の誓約書を提出せず、又
は虚偽の誓約をし、若しくは誓約書に反する
こととなった場合は、当該者の入札を無効と
する。
(5)担当者等から提出される契約関係書類につ
いては、事業者としての決定であること。ま
た、押印が省略された契約関係書類に虚偽記
載等の不正が発覚した場合は、契約解除や違
約金を徴取する場合がある。

(7)落札者の決定方法予算決算及び会計令第
79条の規定に基づいて作成された予定価格の
制限の範囲内で最低価格をもって有効な入札
を行った者を落札者とする。
(8)契約締結日までに国の予算(暫定予算を含
む。)が成立していない場合は、契約締結日は
予算が成立した日以降とする。また暫定予算
となった場合は、契約内容等について変更が
生じる可能性や、本事業に係る予算の決定状
況によって仕様の内容について変更が生じる
可能性があるので、その際は双方で別途協議
する。

those who shall
①Not come under Article 70 of the Cabi-
net Order concerning the Budget, Audit-
ing and Accounting. Furthermore, mi-
nors, Persons under Conservatorship or
Persons under Assistance who have ob-
tained the consent necessary for conclud-
ing a contract may be applicable under
cases of special reasons within the said
clause.
②Not come under Article 71 of the Cabi-
net Order concerning the Budget, Audit-
ing and Accounting.
③ Have 「A」, 「C on
"Manufacture of product" or "Sale of pro-
duct" in terms of qualification for partici-
pating in tenders by Ministry of Health,
Labour and Welfare (Single qualification
for every ministry and agency) in the fis-
cal years of 2025, 2026 and 2027.
④ Not be in arrears either of insurance
premiums (2 insurance year about (e) and
(f)) for the last two years of the this bid
submission deadline, for bidders subject
to the following insurance systems
(a) Welfare pension insurance
(b) Health insurance (which is managed
by Japan Health Insurance Asso-
ciation)
(c) Seamen's insurance
(d) National pension
(e) Worker's compensation insurance
(f) Unemployment insurance
⑤ Prove to have no false statement in
tendering documents.
⑥ Prove neither the business condition
nor credibility is deteriorating
⑦Not in duties during a period taking a
nomination stop from Ministry of Health,
Labour and Welfare.
⑧ Not received any administrative sanc-
tions for violation of the Laws and Regu-
lations under the jurisdiction Ministry of
Health, Labour and Welfare within 1
year. However, this shall not apply in the
case where the supplier took corrective
actions and received "Notification of
withdrawing for the order of suspension
of use" after received an order of suspen-
sion of use by Labour Standard Inspec-
tion Administration, due to violation of
the Labour Standard related laws and re-
gulations (*)
⑨Not be sent the documents to the public
prosecutor's office due to violation of the
Laws and Regulations under the jurisdic-
tion Ministry of Health, Labour and Wel-
fare and be made this fact public.
⑩ Meet the qualification requirements
which the Obligating Officer may specify
