次のとおり一般競争入札に付します。

1 調達内容

(2) 購入等件名及び数量 眼撮影装置一式

による。

(6) 入札方法 上記1(2)の件名のほか納入に要
する一切の諸経費を含めた額で行う。なお、
落札決定にあたっては、入札書に記載された
金額に当該金額の10パーセントに相当する額
を加算した金額（当該金額に1円未満の端数
があるときは、その端数金額を切り捨てるも
のとする。）をもって落札価格とするので、入
札者は、消費税及び地方消費税に係る課税事
業者であるか免税事業者であるかを問わず、
見積もった契約金額の110分の100に相当する
金額を入札書に記載すること。
2 競争参加資格
(1) 予算決算及び会計令第70条の規定に該当し
ない者であること。ただし、未成年者、被保
佐人、又は被補助人であって、契約締結のた
めに必要な同意を得ている者については、こ
の限りではない。
(2) 予算決算及び会計令第71条の規定に該当し
ない者であること。
(3) 厚生労働省競争参加資格（全省庁統一参加
資格）において「物品の販売」のA、B、C
又はDの等級に格付けされ、九州・沖縄地域
の競争参加資格を有する者であること。
(4) 薬事法に基づく医療器具の販売業の届出を
していることを証明した者であること。
(5) その他契約担当役が必要と定める資格を有
する者であること。

(6) Qualification for participating in the tendering procedures: Suppliers eligible for participating in the proposed tender are those who shall:
① not come under Article 70 of the Cabinet Order concerning the Budget, Auditing and Accounting, Furthermore, mi-
nors, Person under Conservatorship or Person under Assistance that obtained the consent necessary for concluding a contract may be applicable under cases of special reasons within the said clause,
② not come under Article 71 of the Cabinet Order concerning the Budget, Auditing and Accounting,
③ have Grade A or B or C or D in “Sales of products” for participating in tenders by Ministry of Health, Labour and Welfare (Single qualification for every ministry and agency) in Kyusyu-Okinawa Region,
④ prove to have registered with the relevant authorities, in accordance with the Pharmaceutical Affairs Law, to initiate business of selling medical appliances,
⑤ meet the qualification requirements which the Obligator Officer may specify,
