(6) 入札方法
① 落札決定は、総合評価落札方式をもって
行うので、提案にかかる性能、機能、技術
等に関する書類（以下「総合評価のための
書類」という。）を提出すること。
② 入札金額については、購入物品のほか、
納入に要する一切の費用を含めた額とする
こと。
なお、落札決定に当たっては、入札書に記載された金額に当該金額の10パーセントに相当する額を加算した金額（当該金額に1円未満の端数があるときは、その端数金額を切り捨てるものとする。）をもって落札価格とするので、入札者は、消費税に係る課税事業者であるか免税事業者であるかを問わず、見積もった契約金額の110分の100に相当する金額を入札書に記載すること。
2 競争参加資格
(1) 独立行政法人国立病院機構契約事務取扱細則（以下、契約細則という。）第5条の規定に該当しない者であること。なお、未成年者、
被保佐人又は被補助人であっても、契約締結のために必要な同意を得ているものは、同条中、特別の理由がある場合に該当する。
(2) 契約細則第6条の規定に該当しない者であること。
(3) 厚生労働省競争参加資格（全省庁統一資格）
「物品の製造」又は「物品の販売」のA、B
又はCの等級に格付され、東海・北陸地域の
競争参加資格を有する者であること。なお、
当該競争参加資格については、令和6年3月
29日付け号外政府調達第58号の官報の競争参
加者の資格に関する公示の別表に掲げる申請
受付窓口において随時受け付けている。
(4) 医薬品、医療機器等の品質、有効性及び安全性の確保等に関する法律に基づく医療機器の販売業の届出をしていることを証明した者であること。
(5) 購入物品に係る迅速なアフターサービス・メンテナンスの体制が整備されていることを証明した者であること。
(6) 契約細則第4条の規定に基づき、経理責任者が定める資格を有する者であること。

(6) Qualification for participating in the tendering procedures: Suppliers eligible for participating in the proposed tender are those who shall:
① not come under Article 5 of the Regulation concerning Contract for National Hospital Organization (NHO). Furthermore, minors, Person under Conservatorship or Person under Assistance that obtained the consent necessary for concluding a contract may be applicable under cases of special reasons within the said clause;
② not come under Article 6 of the Regulation concerning Contract for NHO;
③ have Grade A, B or C in “Manufacturers of products” or “Sales of products” for participating in tenders by Ministry of Health, Labour and Welfare (Single qualification for every ministry and agency) in Tokai-Hokuriku Region;
④ prove to have registered with the relevant authorities, in accordance with the Pharmaceutical Affairs Law, to initiate business of selling medical appliances;
⑤ prove to have prepared a system to provide rapid after-sale service and maintenance for the procured products;
⑥ meet the qualification requirements which the Obligating Officer may specify in accordance with Article 4 of the Regulation;
