記載心得
1 本籍欄には、外国人にあっては、国籍を記載すること。
2 年齢18年以上の船員については、年齢18年に達する日欄に斜線を引くこと。
3 国際トン数証書又は国際トン数確認書の交付を受けている日本船舶にあっては、総トン数に国際総トン数を付記すること。
4 職務欄には、「一等航海士」、「操機手」等と記載し、同一職名の船舶職員が2名以上乗り組むときは、その順位（「首席」、「次席」等）を付記すること。また、通信士については、無線電信又は無線電話の別を付記すること。
衛生管理者、救命艇手、安全担当者、消火作業指揮者若しくは衛生担当者に選任された者、船舶料理士、航海当直部員又は危険物等取扱責任者については、その旨（救命艇手のうち限定救命艇手である者については、その旨）を記載すること。
5 本邦外の地域へ赴く航海に従事する船舶に乗り組む船員については、船名及び職務（衛生管理者、救命艇手、安全担当者、消火作業指揮者、衛生担当者、船舶料理士、航海当直部員及び危険物等取扱責任者を除く。）に英訳を付すること。
6 区分欄には、雇入、雇止、更新又は変更の別を記載すること。雇止、更新又は変更の場合にあっては、雇入時に交付した書面又はその写しに追記すること。
7 船舶所有者、船員又は船舶の情報に記載事項に変更があったときは、変更前の記載事項を読み得るように抹消して訂正し、その箇所に変更年月日を付記すること。また、船名、船舶の用途、総トン数、航行区域若しくは従業制限若しくは従業区域、主機の種類若しくは出力、職務又は雇入期間の変更については、変更内容を備考欄に記載すること。
8 妊産婦の船員については、その旨（「妊娠中、何年何月何日出申」、「産後、何年何月何日出産」等）を備考欄に記載すること。
9 船長は、船舶所有者が、船員が下船する際に雇入契約の終了の届出をすることができないときは、その事由及び自らの氏名を備考欄に記載すること。
10 備考欄には、上記のほか、船員が希望したときは、給料その他の労働条件、雇止事由等を記載すること。
How to enter
1 Write the nationality in the column "Nationality" in the case of a foreigner.
2 For a seafarer 18 or more years of age, a slash shall be drawn in the column of "Date When the Age Will Reach 18 Years Old".
3 In case of a Japanese ship which has an international tonnage certificate or a tonnage certificate, international gross tonnage shall be added in the column of "Gross Tonnage".
4 In the column of "Position", "Chief mate" or "Oiler", etc., shall be entered, and when two or more officers are aboard in the same position, the order ("The head", "The next", etc.) shall be added in front of their position. In the case of a radio operator, the difference between radiotelegraph and radiotelephony shall be added. In the case of a health supervisor, lifeboatman (or person with designated inflatable liferaft), safety manager, fire-fighting leader, health manager, ship's cook, rating forming part of a navigational watch/an engine-room watch or person assigned with responsibilities for handling dangerous and other substances, such positions shall be entered.
5 For the seafarer boarding a ship engaged on an international voyage, if the seafarer's language is not English, the "Name of Ship" and "Position" (excluding health supervisor, lifeboatman, safety manager, fire-fighting leader, health manager, ship's cook, rating forming part of a navigational watch/an engine-room watch or person assigned with responsibilities for handling dangerous and other substances) shall include a translation into English.
6 In the column of "Distinction", indicate whether it is engagement, termination, renewal, or amendment. For termination, renewal, or amendment, make an additional entry on the document or a copy thereof that was issued at the time of engagement.
7 When there is a change in the particulars of the shipowner, seafarer, or ship, the previous entry shall be crossed out and the change shall be entered, together with the date of the change. For changes to the name of ship, type of ship, gross tonnage, navigation area, fishing restriction, fishing area, type or power of main propulsion machinery, position, or period of the articles of agreement, the details of the change shall be noted in the remarks column.
8 For the pregnant seafarer, the event (ex. "Pregnant, notified on dd/mm/yyyy", "After childbirth, delivered on dd/mm/yyyy", etc.) shall be entered in the column of "Remarks".
9 In case the shipowner cannot submit the notification of termination of the employment contract when the seafarer disembarks, the master shall enter the reason and his own name in the column of "Remarks".
10 In the column of "Remarks", when the holder of this pocket ledger requests, wages and other labor conditions or reasons for the termination of the articles of agreement, etc., can be entered.
