(4)入札の無効本公告に示した競争参加資格
のない者の提出した入札書、入札者に求めら
れる義務を履行しなかった者の提出した入札
書、その他入札の条件に違反した者の提出し
た入札書は無効とする。(3)の誓約書を提出せ
ず、又は虚偽の誓約をし、若しくは誓約書に
反することとなった場合は、当該者の入札を
無効とする。
(5)担当者等から提出される契約関係書類につ
いては、事業者としての決定であること。ま
た、押印が省略された契約関係書類に虚偽記
載等の不正が発覚した場合は、契約解除や違
約金を徴取する場合がある。
(6)契約書作成の要否要
(7)予算決算及び会計令第79条の規定に基づい
て作成された予定価格の制限の範囲内で最低
価格をもって有効な入札を行った者を落札者
とする。ただし、落札者となるべき者の入札
価格によっては、その者により当該契約の内
容に適合した履行がなされないおそれがある
と認められるとき、又はその者と契約を締結
することが公正な取引の秩序を乱すこととな
るおそれがあって著しく不適当であると認め
られるときは、予定価格の制限の範囲内の価
格をもって入札した他の者のうち最低の価格
をもって入札した者を落札者とすることがあ
る。

6 Summary
(1)Official in charge of disbursement of the

(4) Term of a contract: From September 8,
2025 through March 11,2026
(5) Delivery Place: The place specified by
the contracting entity.
(6)Qualifications for participating in the
tendering procedures: suppliers eligible for
participating in the proposed tender are
those who shall:
①not come under Article 70 of the Cabi-
net Order concerning the Budget, Audit-
ing and Accounting. Furthermore, mi-
nors, Persons under Conservatorship or
persons under Assistance who have ob-
tained the consent necessary for conclud-
ing a contract May be applicable under
cases of special reasons within the said
clause.
②not come under Article 71 of the Cabi-
net Order concerning the Budget, Audit-
ing and Accounting.
③have Grade A or B or C, on "Manufac-
ture of product" in terms of the qualifica-
tions for participatin-
istry of Health, Labour and Welfare
(Single qualification for every ministry
and agency) in the fiscal year of 2025,
2026 and 2027.
④For bidders subject to the following in-
surance system of each item, and it is not
in arrears either of insurance premiums
(2 insurance year about e and f ) for the
last two years of the bid submission dead-
line of this bid :
a Welfare annuity insurance
b Health insurance (the thing witch as-
sociation of national health insurance
has jurisdiction over)
c Seamen's insurance
d National pension
e Worker's compensation insurance

⑤prove to have no false statement in ten-
dering documents
⑥prove neither the business condition
nor credibility is considerably dete-
riorating
⑦Do not go out of Ministry of Health
Labour and Welfare about duties during
a period taking a nomination stop.
⑧ Not anc-
tions for violation of the Laws and Regu-
lations under the jurisdiction Ministry of
Health, Labour and Welfare within 1
year. However, this shall not apply in the
case where the supplier took corrective
actions and received "Notification of
withdrawing for the order of suspension
of use" after received an order of suspen-
sion of use by Labour Standard Inspec-
tion Administration, due to violation of
the Labour Standard related laws and re-
gulations (*).
⑨Not be sent the documents to the public
prosecutor's office due to violation of the
Laws and Regulations under the jurisdic-
tion Ministry of Labour, Health and Wel-
fare and be made this fact public.
⑩ Meets the qualification requirements
which the Obligating Officer may specify
in accordance with Article 73 of the Cabi-
net Order concerning the Budget, Audit-
ing and Accounting.
