ただし、労働基準関係法令違反(※)によ
り労働基準監督機関から使用停止等命令を受
けたが、是正措置を行い「使用停止等命令解

(4)留意事項
ア入札者から提出される契約関係書類につ
いては、事業者としての決定であることを
要する。
イ契約関係書類に虚偽記載等の不正が発覚
した場合は、契約解除や違約金を徴収する
ことがある。
(5)入札の無効上記2に示した競争参加資格
を有しない者のした入札及び入札に関する条
項に違反した入札は無効とする。
また、(3)の誓約書を提出せず又は虚偽の誓
約をし、若しくは誓約書に反することとなっ
たときは、当該者の入札を無効とする。

tendering procedures: Supplier eligible for
participating in the proposed tender are
those who shall:
①Have Grade "A" or "B" in the public
tender qualification category of "Infor-
mation processing" or "Software develop-
ment" among "Offer of services, etc" by
the Ministry of Health, Labour and Wel-
fare (The common qualification for all
ministries and agencies) in the fiscal
years of 2025, 2026 and 2027.
②Not come under Article 70 of the Cabi-
net Order concerning the Budget, Audit-
ing and Accounting. Furthermore, mi-
nors, Person under Conservatorship or
Person under Assistance that obtained
the consent necessary for concluding a
contract may be applicable under cases
of special reasons within the said clause
③ Not come under Article 71 of the Cabi
net Order concerning the Budget, Audit-
ing and Accounting
④ Prove to have no false statement in
tendering documents.
⑤ Prove neither the business condition
nor credibility is deteriorating.
⑥For bidders subject to the following in-
surance system of each item, and it is not
in arrears either of insurance premiums
