(5)入札方法。入札金額は総価で行う.
なお、落札決定に当たっては、入札書に記
載された金額に当該金額の10パーセントに相
当する額を加算した金額(当該金額に1円末
満の端数があるときは、その端数金額を切り
捨てるものとする。)をもって落札価格とする
ので、入札者は、消費税に係る課税事業者で
あるか免税事業者であるかを問わず、見積
もった契約金額の110分の100に相当する金額
を入札書に記載すること。
(6)調達ポータルの利用本案件は調達ポータ
ルで行う。
なお、調達ポータルによりがたい者は、書
面により申し出た場合に限り、紙入札方式に
代えることができる。紙入札方式の場合につ
いては、郵送にて厚生労働省競争参加資格(全
省庁統一資格)及び入札書の提出を行う。
2競争参加資格
(1)予算決算及び会計令第70条の規定に該当し
ない者であること。
なお、未成年者、被保佐人又は被補助人で
あって契約締結のために必要な同意を得てい
る者は、同条中、特別の理由がある場合に該
当する。
(2)予算決算及び会計令第71条の規定に該当し
ない者であること。
(3)令和7・8・9年度厚生労働省競争参加資
格(全省庁統一資格)において、「物品の製造」
のうち営業品目が「フォーム印刷」又は「そ
の他印刷類」を有しており、「A」、「B」又は
「C」等級のいずれかの等級に格付けされて
いる者であること。
(4)資格審査申請書又は添付書類に虚偽の事実
を記載していないと認められる者であるこ
と.
(5)経営の状況又は信用度が極度に悪化してい
ないと認められる者であること。
(6)次の各号に掲げる制度が適用される者に
あっては、この入札の入札書提出期限の直近
2年間(オ及びカについては2保険年度)の
保険料について滞納がないこと。
ア厚生年金保険、イ健康保険(全国健康
保険協会が管掌するもの)、ウ船員保険、
エ国民年金、オ労働者災害補償保険、カ
雇用保険
(7)当該物品又はこれと類似の物品を相当量製
造納入した実績を有する者であること。
(8)必要な用紙を確実に入手できると認められ
る者であること。
(9)本調達において当方で指定した試作品を事
前に提出し、当方が実施する試作品試験に合
格した者であること。
(10)過去1事業につき実施する日本銀行及びゆ
うちょ銀行のOCR帳票試験を受験し、合格
した実績があること。
(11)厚生労働省から指名停止を受けている期間
中の者でないこと。
(12)過去1年以内に、厚生労働省労働基準局の
所管する委託事業において、以下のいずれか
に該当し、信頼関係を築くことが困難であっ
て、当該委託業務の遂行に支障を来すと判断
されるものでないこと。
ア契約書に基づき、受託者の責において、
委託事業の全部若しくは一部の停止、又は
契約の解除を受けたこと。
イ契約書に基づき、委託者による監査を受
け、業務実施に係る指導を受けたにもかか
わらず、期日までに改善をしなかったこと。
ウ契約書に基づき、委託者から委託事業実
施状況報告書の提出を求められたにも関わ
らず、期日までに回答をしない又は回答が
不十分など誠実に対応しなかったこと。
エ契約書に基づく検査の結果、受託者の責
において、業務の未履行のために不合格と
なったこと。
(13)その他予算決算及び会計令第73条の規定に
基づき、支出負担行為担当官が定める資格を
有する者であること。
ア過去1年以内に厚生労働省所管法令違反
により行政処分を受けていないこと。ただ
し、労働基準関係法令違反(※)により労
働基準監督機関から使用停止等命令を受け
たが、是正措置を行い「使用停止等命令解
除通知書」を受理している場合には、この
限りではない。
イ過去1年以内に厚生労働省所管法令違反
により送検され、この事実を公表されてい
ないこと。
ウ過去1年以内に厚生労働省所管法令に基
づく公表制度により、又は違法な長時間労
働や過労死等が複数の事業場で認められた
企業として、平成29年1月20日付け基発
0120第1号厚生労働省労働基準局長通達
「違法な長時間労働や過労死等が複数の事
業場で認められた企業の経営トップに対す
る都道府県労働局長等による指導の実施及
び企業名の公表について記3に基づく企
業名の公表をされていないこと。
※労働基準法、労働安全衛生法、最低賃金
法、賃金の支払の確保等に関する法律、家
内労働法、作業環境測定法、じん肺法、炭

参加する者は、製造物品の品質、特質及び条
件に適合する試作品を、上記3(4)に示した期
限までに提出場所へ提出しなければならな
い。入札者の試作品は当省において審査し、
採用し得ると判断された場合の入札書のみを
落札決定の対象とする。

なお、入札者は、支出負担行為担当官が指
定する暴力団等に該当しない旨を記した誓約
書を提出しなければならない。
また、入札者は、開札日の前日までの間に
おいて、支出負担行為担当官から当該書類等
に関し説明を求められた場合は、それに応じ
なければならない。
(4)入札時留意事項
ア入札者から提出される入札・契約関係書
類については、事業者としての決定である
ことを要する。
イ入札・契約関係書類に虚偽記載等の不正
が発覚した場合は、契約解除や違約金を徴
取することがある。
(5)入札の無効上記2に示した参加競争資格
のない者の提出した入札書、上記(3)に示した
入札者に要求される事項を履行しなかった者
の提出した入札書、その他入札の条件に違反
した者の提出した入札書は無効とする。
(6)契約書作成の要否落札者との契約締結に
あたり、会計法第29条の8の規定により契約
書の作成を要する。
(7)落札者の決定方法本公告に示した業務を
履行できると支出負担行為担当官が判断した
入札者であって、予算決算及び会計令第79条
の規定に基づいて作成された予定価格の制限
の範囲内で最低価格をもって有効な入札を
行った者を落札者とする。

of qualification for participating in ten-
ders by Ministry of Health, Labour and
Welfare (Single qualification for every
ministry and agency) in the fiscal years

②Not come under Article 70 of the Cabi-
net Order concerning the Budget, Audit-
ing and Accounting. Furthermore, mi-
nors, Person under Conservatorship or
Person under Assistance that obtained
the consent necessary for concluding a
contract may be applicable under cases
of special reasons within the said clause.
③Not come under Article 71 of the Cabi-
net Order concerning the Budget, Audit-
ing and Accounting.
④ Prove to have no false statement in
tendering documents.
⑤ Prove neither the business condition
nor credibility is deteriorating.
