「自己申告書」、別紙6の暴力団等に該当し
ない旨の「誓約書」及び別紙7「保険料納付
に係る申立書」その他入札説明書に定める提
出書類を提出しなければならない。入札者の
競争参加資格に関する証明書等は、当労働局
において審査するものとし、採用しうると判
断された場合の入札書のみを落札決定の対象
とする。入札者は、支出負担行為担当官から
当該書類に関し説明を求められた場合は、そ
れに応じなければならない。
(4)入札の無効本公告に示した競争参加資格
のない者の提出した入札書、入札者に求めら
れる義務を履行しなかった者の提出した入札
書は無効とする。また入札に参加した者が、
上記4(3)の誓約書を提出せず、又は虚偽の誓
約をし、若しくは誓約書に反することとなっ
たときは、当該者の入札を無効とするものと
する。
(5)契約書作成の要否要
原則、電子契約によること。
(6)落札者の決定方法本公告に示した物品を
納入できると支出負担行為担当官が判断した
者であって、予算決算及び会計令(昭和22年
勅令第165号)第79条の規定に基づいて作成
された予定価格の制限の範囲内で最低価格を
もって有効な入札を行った入札者を落札者と
する。

業者としての決定であること。

ratorship or assistance that obtained the
consent required for concluding a con-
tract may be applicable under cases of
special reasons within the said clause,
②Not covered by Article 71 of the Cabi-
net Order concerning the Budget, Audit-
ing and Accounting,
③ Have Grade "A" or "B" on "provision
of services" in terms of qualifications for
participating in tenders by the Ministry
of Health, Labor and Welfare (Inter-
Ministerial, Inter-Agency Standardized
Qualification) in the fiscal year of 2025,
2026 and 2027.
④ Be able to prove to have no false state-
ment in the tendering documents or its at-
tached documents to be submitted.
⑤Be able to prove neither the business
condition nor credibility is considerably
deteriorating,
⑥Not be under the period of business
suspension order issued by the Ministry
of Health, Labor and Welfare,
⑦Not be behind in payment of the pre-
mium in the last two years (the latest two
insurance years as for (5) and (6)), such
as:
(1) Employees' Pension Insurance
(2) Health Insurance (managed by the
Japan Health Insurance Association)
(3) Seamen's Insurance
(4) National Pension
(5) Industrial Accident Compensation
Insurance
(6) Insurance
⑧Within the past year, not to be subject
to administrative disposition by violation
of laws under the jurisdiction of the Min-
istry of health, Labor and Welfare,
⑨ Meet the qualification requirements
stipulated by the Director of General
Coordination Department in accordance
with Article 73 of the Cabinet Order con-
cerning the Budget, Auditing and Ac-
counting.
