ることとするが、提案書類の提出期限までに

(4) 入札説明書の交付方法 本公告開始日から

電子メールでの交付を希望する場合は、同

交付の申込みは、期限までに到着するよう
に送付しなければならない。未着の場合、そ
の責任は参加者に属するものとし、期限内の
送付がなかったものとみなす。
4 入札書の提出場所等

(3) 入札書の提出方法 入札書の提出に当たっ
ては、原則、電子調達システムで行う。
なお、電子調達システムによりがたい者は、
支出負担行為担当官に申し出た場合に限り紙
入札方式に変えることができる。紙入札方式
により提出する場合は封筒に入れ、かつ、そ
の封皮に氏名(法人の場合はその名称又は商
号)、あて名(支出負担行為担当官厚生労働
省労働基準局労災管理課長あて)及び「令和
8年4月16日開札[個人事業者等の業務上災
害報告制度の新設に伴う「労働安全衛生法関
係の届出・申請等帳票印刷に係る入力支援
サービス」の改修業務]入札書在中」と朱記
しなければならない。
再度入札となることも考慮し、必要に応じ
て複数枚の入札書を提出すること(封筒に、
必要事項の他、何回目の入札書であるかを必
ず明記する)。
(4) 開札の日時 令和8年4月16日12時00分
応札者は立ち会わないこととし、入札結果
については応札者全員にメール等で連絡す
る。
5 その他
(1) 契約手続きにおいて使用する言語及び通貨
日本語及び日本国通貨に限る。
(2) 入札保証金及び契約保証金 免除
(3) 入札者に要求される事項 この入札に参加
を希望する者は、上記2の競争参加資格を有
することを証明する書類を令和8年4月1日
10時00分までに提出しなければならない。
また、入札に参加を希望する者は、上記証
明書類と合わせて暴力団等に該当しない旨の
誓約書を提出しなければならない。
入札者は、支出負担行為担当官から当該書
類に関し説明を求められた場合は、これに応
じなければならない。
(4) 入札の無効 本公告に示した競争参加資格
のない者の提出した入札書、入札者に求めら
れる義務を履行しなかった者の提出した入札
書は無効とする。(3)の誓約書を提出せず、又
は虚偽の誓約をし、若しくは誓約書に反する
こととなった場合は、当該者の入札を無効と
する。
(5) 担当者等から提出される契約関係書類につ
いては、事業者としての決定であること。ま
た、押印が省略された契約関係書類に虚偽記
載等の不正が発覚した場合は、契約解除や違
約金を徴収する場合がある。

(7) 落札者の決定方法 予算決算及び会計令第
79条の規定に基づいて作成された予定価格の
制限の範囲内であり、入札説明書等で指定す
る技術等の要件のうち必須とされた項目の最
低限の要求要件をすべて満たしている提案を
した入札者の中から、入札説明書等で定める
総合評価落札方式の方法をもって落札者の決
定をする。ただし、落札者となるべき者の入
札価格によっては、その者により当該契約の
内容に適合した履行がなされないおそれがあ
ると認められるとき、又はその者と契約を締
結することが公正な取引の秩序を乱すことと
なるおそれがあって著しく不適当であると認
められるときは、予定価格の制限の範囲内の
価格をもって入札した他の者のうち、総合評
価点の最も高い者を落札者とすることがあ
る。
(8) 契約締結日までに国の予算(暫定予算を含
む。)が成立していない場合は、契約締結日は
予算が成立した日以降とする。また暫定予算
となった場合は、契約内容等について変更が
生じる可能性や、本事業に係る予算の決定状
況によって仕様の内容について変更が生じる
可能性があるので、その際は双方で別途協議
する。

6 Summary

(6) Qualifications for participating in the
tendering procedures : suppliers eligible for
participating in the proposed tender are
those who shall :
① Not come under Article 70 of the Cabi-
net Order concerning the Budget, Audit-
ing and Accounting. Furthermore, mi-
nors, Persons under Conservatorship or
Persons under Assistance who have ob-
tained the consent necessary for conclud-
ing a contract may be applicable under
cases of special reasons within the said
clause.
② Not come under Article 71 of the Cabi-
net Order concerning the Budget, Audit-
ing and Accounting.
③ Have either Grade "A", "B" or "C" on
"Offer of services, etc" in terms of qualifi-
cation for participating in tenders by Mi-
nistry of Health, Labour and Welfare
(Single qualification for every ministry
and agency) in the fiscal years of 2025,
2026 and 2027.
④ For bidders subject to the following in-
surance system of each item, and it is not
in arrears either of insurance premiums
(2 insurance year about (e) and (f)) for the
last two years of the bid submission dead-
line of this bid :
(a) Welfare annuity insurance
(b) Health insurance (the thing which as-
sociation of national health in surance
has jurisdiction over)
(c) Seamen's insurance
(d) National pension
(e) Worker's compensation insurance
(f) Unemployment insurance
⑤ Prove to have no false statement in
tendering documents.
⑥ Prove neither the business condition
nor credibility is deteriorating.
⑦ Do not go out of Ministry of Health,
Labour and Welfare about duties during
a period taking a nomination stop.
⑧ Not received any administrative sanc-
tions for violation of the Laws and Regu-
lations under the jurisdiction Ministry of
Health, Labour and Welfare within 1
year. However, this shall not apply in the
case where the supplier took corrective
actions and received "Notification of
withdrawing for the order of suspension
of use" after received an order of suspen-
sion of use by Labour Standard Inspec-
tion Administration, due to violation of
the Labour Standard related laws and re-
gulations (*).
⑨ Not be sent the documents to the public
prosecutor's office due to violation of the
Laws and Regulations under the jurisdic-
tion Ministry of Health, Labour and Wel-
fare and be made this fact public.
