による。

(6)入札方法入札金額については単価を記入
すること。
なお、落札決定に当たっては、入札書に記
載された金額に当該金額の10パーセントに相
当する額を加算した金額(当該金額に1円末
満の端数があるときは、その端数金額を切り
捨てるものとする。)をもって落札価格とする
ので、入札者は、消費税に係る課税事業者で
あるか免税事業者であるかを問わず、見積
もった契約金額の110分の100に相当する金額
を入札書に記載すること。
(7)その他独立行政法人国立病院機構契約事
務取扱細則(以下、契約細則という。)第22条
の規定に基づき単価契約とする。
2競争参加資格
(1)契約細則第5条の規定に該当しない者であ
ること。なお、未成年者、被保佐人又は被補
助人であっても、契約締結のために必要な同
意を得ているものは、同条中、特別の理由が
ある場合に該当する。
(2)契約細則第6条の規定に該当しない者であ
ること。
(3)入札者の競争参加資格厚生労働省競争参
加資格(全省庁統一資格)「物品の販売」のA、
B、C又はDの等級に格付され、関東甲信越
地域の競争参加資格を有する者であること。
なお、当該競争参加資格については、令和7
年3月31日付け号外政府調達第57号の官報の
競争参加者の資格に関する公示の別表に掲げ
る申請受付窓口において随時受け付けてい
る。
(4)医薬品、医療機器等の品質、有効性及び安
全性の確保等に関する法律に基づいて医薬品
の一般販売業の許可を受けていることを証明
した者であること。
(5)契約細則第4条の規定に基づき、経理責任
者が定める資格を有する者であること。
3入札書の提出場所等

(3)入札者に要求される事項この一般競争に
参加を希望する者は、封印した入札書に本公
告に示した物品を納入できることを証明する
書類を添付して入札書の受領期限までに提出
しなければならない。入札者は、開札日の前
日までの間において、経理責任者から当該書
類に関し説明を求められた場合は、それに応
じなければならない。
(4)入札の無効本公告に示した競争参加資格
のない者の提出した入札書、入札者に求めら
れる義務を履行しなかった者の提出した入札
書は無効とする。

(6)落札者の決定方法本公告に示した物品を
納入できると経理責任者が判断した資料を添
付して入札書を提出した入札者であって、契
約細則第21条の規定に基づいて作成された予
定価格の制限の範囲内で最低価格をもって有
効な入札を行った入札者を落札者とする。

(6)Qualification for participating in the ten-
dering procedures: Suppliers eligible for
participating in the proposed tender are
those who shall:
A not come under Article 5 of the Regula-
tion concerning Contract for National
Hospital Organization (NHO). Further-
more, minors, Person under Conservator-
ship or Person under Assistance that ob-
tained the consent necessary for conclud-
ing a contract may be applicable under
cases of special reasons within the said
clause :
B not come under Article 6 of the Regula-
tion concerning Contract for NHO;
C have the following Grade qualification,
the Grade A, B, C or D in "Sales of prod-
ucts" for participating in tenders by Min-
istry of Health, Labour and Welfare
(Single qualification for every ministry
and agency) in Kanto-Koshinetsu
Region;
D prove to have registered with the rel-
evant authorities, in accordance with the
Pharmaceutical Affairs Law, to initiate
business of selling medical appliances;
E prove that they can deliver the drugs
on the date and to the place specified by
the Obligating Officer ;
F meet the qualification requirements
which the Obligating Officer may specify
in accordance with Article 4 of the Regu-
lation :
