(6)入札方法 入札金額は総価で行う.
なお、落札決定に当たっては、入札書に記
載された金額に当該金額の10パーセントに相
当する額を加算した金額(当該金額に1円未
満の端数があるときは、その端数金額を切り
捨てるものとする。)をもって落札価格とする
ので、入札者は、消費税に係る課税事業者で
あるか免税事業者であるかを問わず、見積
もった契約金額の110分の100に相当する金額
を入札書に記載すること。
2競争参加資格
(1)令和07・08・09年度厚生労働省競争参加資
格(全省庁統一資格)において、「物品の製造」
のうち営業品目が「フォーム印刷」を有して
おり、A、B又はC等級に格付されている者
であること。
(2)予算決算及び会計令第70条の規定に該当し
ない者であること。
なお、未成年者、被保佐人又は被補助人で
あって、契約締結のための必要な同意を得て
いる者は、競争入札に参加することができる。
(3)予算決算及び会計令第71条の規定に該当し
ない者であること。
(4)資格審査申請書又は添付書類に虚偽の事実
を記載していないと認められる者であるこ
と。
(5)経営の状況又は信用度が極度に悪化してい
ないと認められる者であること。
(6)次の各号に掲げる制度が適用される者に
あっては、この入札の入札書提出期限の直近
2年間((5)及び(6)については2保険年度)の
保険料について滞納がないこと。
①厚生年金保険、②健康保険(全国健康保
険協会が管掌するもの)、③船員保険、④国
民年金、⑤労働者災害補償保険、⑥雇用保険
(7)過去5年以内に当該印刷物又は類似の物
(3枚複写式OCR様式)を150万組以上製
造した実績を有するもの,
(8)必要な用紙を確実に入手することが出来る
と認められる者であること。
(9)当方で指定した試作品を事前に提出した者
であること。ただし、入札に参加できるのは、
当方で実施する試作品試験に合格した者に限
る。
(10)厚生労働省から指名停止を受けている期間
でないこと。
(11)過去1年以内に厚生労働省所管法令違反に
より行政処分を受けていないこと。ただし、
労働基準関係法令違反(※)により労働基準
監督機関から使用停止等命令を受けたが、是
正措置を行い「使用停止等命令解除通知書」
を受理している場合には、この限りではない。
(12)過去1年以内に厚生労働省所管法令違反に
より送検され、この事実を公表されていない
とこ
(13)過去1年以内に厚生労働省所管法令に基づ
く公表制度により、又は違法な長時間労働や
過労死等が複数の事業場で認められた企業と
して、平成29年1月20日付け基発0120第1号
厚生労働省労働基準局長通達「違法な長時間
労働や過労死等が複数の事業場で認められた
企業の経営トップに対する都道府県労働局長
等による指導の実施及び企業名の公表につい
て記3、平成31年1月25日付け基発0125第
1号「裁量労働制の不適正な運用が複数の事
業場で認められた企業の経営トップに対する
都道府県労働局長による指導の実施及び企業
名の公表について」記の3に基づく企業名の
公表をされていないこと。
(14)その他予算決算及び会計令第73条の規定に
基づき、支出負担行為担当官が定める資格を
有する者であること。

4電子調達システムの利用
本案件は、電子調達システムで行う。
なお、電子調達システムによりがたい者は、
書面により申し出た場合に限り、紙入札方式に
代えることができる。紙入札方式の場合につい
ては、郵送にて厚生労働省競争参加資格(全省
庁統一資格)及び入札書の提出を行う。
5その他
(1)契約手続において使用する言語及び通貨
日本語及び日本国通貨
(2)入札保証金及び契約保証金免除
(3)入札者に要求される事項この一般競争に
参加する者は、製造物品の品質、特質及び条
件に適合する試作品と競争参加資格に関する
証明書等を令和7年9月17日までに提出しな
ければならない。入札者の試作品及び関係書
類は当省において審査するものとし、採用し
得ると判断された場合の入札書のみを落札決
定の対象とする。入札者は、開札日の前日ま
での間において、支出負担行為担当官から当
該書類等に関し説明を求められた場合は、そ
れに応じなければならない。
(4)留意事項
①入札者から提出される契約関係書類につ
いては、事業者としての決定であることを
要する。
②契約関係書類に虚偽記載等の不正が発覚
した場合は、契約解除や違約金を徴収する
ことがある。

のない者の提出した入札書、入札者に求めら
れる義務を履行しなかった者の提出した入札
書その他入札の条件に違反した者の提出した
入札書は無効とする。

履行できると支出負担行為担当官が判断した
入札者であって、予算決算及び会計令第79条
の規定に基づいて作成された予定価格の制限
の範囲内で最低価格をもって有効な入札を
行った者を落札者とする。
ただし、落札者となるべき者の入札価格に
よっては、その者により当該契約の内容に適
合した履行がなされないおそれがあると認め
られるとき又はその者と契約を締結すること
が公正な取引の秩序を乱すこととなるおそれ
があって著しく不適当であると認められると
きは、予定価格の制限の範囲内の価格をもっ
て入札した他の者のうち最低の価格をもって
入札した者を落札者とすることがある。

tendering procedures: Suppliers eligible for
participating in the proposed tender are
those who shall:
①Have Grade A, B or C, on "Form print"
among "Manufacture of product" in terms
of qualification for participating in ten-
ders by Ministry of Health, Labour and
Welfare (Single qualification for every
ministry and agency) in the fiscal years
2025, 2026 and 2027
②Not come under Article70 of the Cabi-
net Order concerning the Budget, Audit-
ing and Accounting. Furthermore, mi-
nors, Person under Conservatorship or
Person under Assistance that obtained
the consent necessary for concluding a
contract may be applicable under cases
of special reasons within the said clause
③ Not come under Article 71 of the Cabi-
net Order concerning the Budget, Audit-
ing and Accounting
④ Prove to have no false statement in
tendering documents.
⑤Prove neither the business condition
nor credibility is deteriorating.
⑥For bidders subject to the following in-
surance system of each item, and it is not
in arrears either of insurance premiums

last two years of the bid submission dead-
