⑹　入札方法　入札金額は、総価（総価は、１円の位まで計上できるものとする。）とし、消費税及び地方消費税を除いた「税抜き額」とすること。仕様書に示す調達内容のほか、手続などに要する一切の費用を含めること。

⑺　本件入札はせり下げ方式による。詳細は入札者に対する指示書のとおり。

２　競争参加資格　本調達に係る競争に参加できる者は、次に掲げる条件をすべて満足し、かつ、契約責任者による本調達に係る競争参加資格確認の結果、資格があると認められた者とする。

⑴　当社ホームページに掲載の「中日本高速道路株式会社契約規則」（平成18年中日本高速道路株式会社規程第25号）第11条の規定に該当しないこと。

⑵　会社更生法に基づく更生手続開始の申立て又は民事再生法に基づく再生手続開始の申立てがなされていないこと。

⑶　業務実績等を証明できること。次の①または②のいずれかに該当する者とする。

①　ＡかつＢの許可取得者

②　Ａの許可取得者でかつＢの許可取得者に業務を委任することが確認できる者

Ａ：特別管理産業廃棄物収集運搬に係る許可　廃棄物の処理及び清掃に関する法律（昭和45年法律第137号）第14条の４第１項の規定による特別管理産業廃棄物収集運搬に関する次の許可を取得

・対象廃棄物：低濃度ＰＣＢ廃棄物

・対象都道府県：愛知県、岐阜県かつ処分場が所在する都道府県

Ｂ：特別管理産業廃棄物処分に係る許可　廃棄物の処理及び清掃に関する法律（昭和45年法律第137号）第15条の４の４の規定による環境大臣の認定を取得または同法第14条の４第６項の規定による特別管理産業廃棄物処分に関する次の許可を取得

・対象廃棄物：低濃度ＰＣＢ廃棄物

⑷　競争参加資格確認申請書等（以下「申請書等」という。）の提出期限の日から開札の日までの期間に、取引停止又は当社ホームページに掲載の「中日本高速道路株式会社工事・調査等の資格登録に関する要領」（平成19年１月10日付け中高契第２号企画本部長通達）に基づき、「地域１」において資格登録停止を受けていないこと。

⑸　本調達の入札に参加した者の間の資本関係又は人的関係がないこと。

⑹　警察当局から、暴力団員等が実質的に経営を支配する者又はこれに準ずる者として、公共工事等からの排除要請があり、当該状態が継続している者でないこと。

設計図書等は、訂正・取消をする場合があり、訂正・取消を行ったときは、入札公告・契約情報検索のページに掲載する。

４　その他

⑴　契約手続において使用する言語及び通貨

日本語及び日本国通貨に限る。

⑵　入札保証金及び契約保証金　免除

⑶　入札者に要求される事項　入札者は、開札日の前日までの間において、契約責任者から申請書等に関し説明を求められた場合は、それに応じなければならない。

⑷　入札の無効

本公告に示した競争参加資格のない者の行った入札、申請書等に虚偽の記載をした者の行った入札及び入札に関する条件に違反した者の行った入札は無効とする。

⑸　契約書作成の要否　要

⑹　落札者の決定方法

契約制限価格の設定はしないが、最低価格をもって有効な入札を行った入札者と入札価格の妥当性や履行の可否を確認するための協議を行い落札者を決定する。

⑹　Qualification for participating in the tendering procedures : Suppliers eligible for participating in the open tender are those who shall

①　not come under Article 11 of the Central Nippon Expressway Companyʼs regulation concerning the contract procedures.

③　have obtained the permission of Article 14－4 Paragraph 1 of the Waste Disposal and Public Cleansing Law (Law No. 137 of 1970).

・Permission for collection and transportation of specially controlled industrial waste (target waste : Low-concentration PCB waste, target prefectures : Aichi prefecture, Gifu prefecture, and disposal point prefectures)

④　Obtained certification from the Minister of the Environment under Article 15－4 Paragraph 4 of the Waste Disposal and Public Cleansing Law (Law No. 137 of 1970) or have obtained the permission of Article 14－4 Paragraph 6 of the same law.

・Permission for disposal of specially controlled industrial waste (Target waste : Low-concentration PCB waste)

※Applicants must meet one of the following conditions :

・A licensee of ⑹③ and ⑹④

・⑹③ licensee and applicants who can be confirmed to delegate duties to a licensee of ⑹④

⑤　not have capital relationship and personnel relationship with other bidders.

⑥　have not received suspension of　registered contractor status or trader after the acceptance of application form for this tender.
