⑹ 入札方法 入札金額は総価で行う。なお、落札決定に当たっては、入札書に記載された金額に当該金額の10パーセントに相当する額を加算した金額（当該金額に１円未満の端数があるときは、その端数金額を切り捨てるものとする。）をもって落札価格とするので、入札者は、消費税に係る課税事業者であるか免税事業者であるかを問わず、見積もった契約金額の110分の100に相当する金額を入札書に記載すること。
２ 競争参加資格
⑴ 予算決算及び会計令第70条の規定に該当しない者であること。なお、未成年者、被保佐人又は被補助人であっても、契約締結のために必要な同意を得ている者は、同条中、特別の理由がある場合に該当する。
⑵ 予算決算及び会計令第71条の規定に該当しない者であること。
⑶ 厚生労働省から指名停止の措置を受けている期間中の者でないこと。
⑷ 令和04・05・06年度厚生労働省競争参加資格（全省庁統一資格）において、厚生労働省大臣官房会計課長から「役務の提供等」でＡ、Ｂ又はＣ等級に格付けされ、関東・甲信越地域の競争参加資格を有する者であること。
⑸ 資格審査申請書又は添付書類に虚偽の事実を記載していないと認められる者であること。
⑹ 経営の状況又は信用度が極度に悪化していないと認められる者であること。
⑺ 社会保険料等（厚生年金保険、健康保険、（全国健康保険協会が管掌するもの）、船員保険、国民年金、労働者災害補償保険及び雇用保険をいう）に加入し、該当する制度の保険料の滞納がないこと。
⑻ 過去１年以内に、厚生労働省所管法令違反により行政処分等を受けていないこと。
⑼ 暴力団が実質的に経営を支配する者又はこれに準ずる者に該当しない旨の誓約書を提出すること。
⑽ その他予算決算及び会計令第73条の規定に基づき、支出負担行為担当官が定める資格を有する者であること。
(11) 入札希望者は「責任あるサプライチェーン等における人権尊重のためのガイドライン」（令和４年９月13日ビジネスと人権に関する行動計画の実施に係る関係府省庁施策推進・連絡会議決定）を踏まえて人権尊重に取り組むよう努めることに誓約したものとする。
(12) 品質管理体制についてＩＳＯ9001：2015、組織としての能力成熟度についてＣＭＭＩレベル３以上のうち、いずれかの認証を受けていること。
(13) プライバシーマーク付与認定、ＩＳＯ／ＩＥＣ27001認証（国際規格）、ＪＩＳ Ｑ 27001認証（日本産業規格）のうち、いずれかを取得していること。
(14) 本業務の作業場所及びデータの保管場所は、日本国内とすること。
(15) 過去３か年分の財務諸表を提出し、経営状態が健全であることを証明すること。また、当該財務諸表には、公認会計士若しくは監査法人による監査報告書の写し、又は、民間で使用されている「中小企業の会計に関する指針の適用に関するチェックリスト」（日本税理士会連合会作成）若しくは「中小企業の会計に関する基本要領の適用に関するチェックリスト」（日本税理士会連合会作成）を用いて税理士が確認した結果の写しを添付すること。
(16) 調達仕様書「11．⑶応札希望者が閲覧すべき資料一覧表」を指定期間内に閲覧すること。
(17) 以下の条件に合致するシステムに関するヘルプデスク業務を担当した実績を有すること。

４ その他
⑴ 契約手続において使用する言語及び通貨

⑶ 入札者に要求される事項 この一般競争に参加を希望する者は、本公告に示した業務が履行できることを証明する書類を指定する期日までに提出しなければならない。入札者は、支出負担行為担当官から当該書類に関し説明を求められた場合は、これに応じなければならない。
また、入札に参加を希望する者は、上記証明書類とあわせて暴力団等に該当しない旨の誓約書を提出しなければならない。
⑷ 入札の無効 本公告に示した競争参加資格のない者の提出した入札書、入札者に求められる義務を履行しなかった者の提出した入札書その他入札の条件に違反した者の提出した入札書は無効とする。
また、入札に参加した者が、⑶の誓約書を提出せず、又は虚偽の誓約をし、若しくは誓約書に反することとなったときは、当該者の入札を無効とするものとする。
⑸ 契約書作成の要否 要
⑹ 落札者の決定方法 次の通り。
① 入札説明書の規定に従い入札者を提出した入札者のうち、競争参加資格及び仕様書の要求要件をすべて満たし、入札説明書において明らかにした性能等の要求要件のうち必須とされた項目の最低限の要求要件を全て満たし、支出負担行為担当官が、業務を適切に履行できると判断した入札者であって、予決令第79条の規定に基づいて作成された予定価格の制限の範囲内で最低価格をもって有効な入札を行った者を落札者とする。
② ①の規定にもかかわらず、落札者となるべき者が次に該当する場合は、最低価格をもって有効な入札を行った他の者のうち、最も価格が低い者を落札者とすることがある。
ア．落札者となるべき者の入札価格によっては、その者により契約の内容に適合した履行がされないおそれがあると認められる場合（低入札価格調査基準額を下回った入札があった場合に開札執行者は、入札者に対して「保留」を宣言し、予決令第86条に規定する調査（契約の内容に適合した履行がなされないおそれがあるか否かについて）を実施した上で落札者を決定し、後日入札者に通知する。）
イ．その者と契約を締結することが公正な取引の秩序を乱すこととなるおそれがあって、著しく不適当であると認められる場合

⑺ Qualifications for participating in the tendering procedures : Suppliers eligible for participating in the proposed tender are those who shall : ①Not come under Article 70 of the Cabinet Order concerning the Budget, Auditing and Accounting. This tender will be conducted through the electronic procurement system. However, those who have difficulty using the electronic procurement system will be allowed to submit paper tenders.
