次のとおり一般競争入札に付します。

１　調達内容

⑹　入札方法　落札者の決定は総合評価落札方式をもって行うので、下記３の⑷に定める期日までに、提案書類等を提出すること。なお、落札決定に当たっては、入札書に記載された金額に当該金額の10％に相当する額を加算した金額（当該金額に１円未満の端数があるときは、その端数金額を切り捨てるものとする。）をもって落札価格とするので、入札者は、消費税及び地方消費税に係る課税事業者であるか免税事業者であるかを問わず、見積もった契約金額の110分の100に相当する金額を入札書に記載すること。

２　競争参加資格

⑴　社会保険診療報酬支払基金一般競争（指名競争）契約の参加者の資格基準第31条に該当しない者であること。

⑵　社会保険診療報酬支払基金一般競争（指名競争）契約の参加者の資格基準第32条に該当しない者であること。

⑶　令和５・６・７年度社会保険診療報酬支払基金一般競争等参加者の資格において、資格の種類が「役務の提供等」であって、「Ａ」、「Ｂ」又は「Ｃ」等級に格付された者、又は当該競争参加者資格を有しない者で、入札説明書の配布までに一般競争等参加者の資格を受け、一般競争等参加資格者名簿に登録され、当該区分及び当該等級に格付された者であること。

⑷　当該業務に対し実績を有すること。

⑸　当該業務に対し確実に実施できると認められるものであること。

①　not come under Article 31 of the qualifications for participating in the Health Insurance Claims Review and Reimbursement Services open tender (or invited tender).

②　not come under Article 32 of the qualifications for participating in the Health Insurance Claims Review and Reimbursement Services open tender (or invited tender).

③　have obtained Grade A, B or C in “offer of services” qualifications for participating in the Health Insurance Claims Review and Reimbursement Services tenders in the fiscal year 2023, 2024 and 2025, or those who shall be qualified as a participant in tenders, registered on the list of qualifying participants, and met the above grade and qualifications by the time of tender.
