(5)入札方法
①入札金額については、納入に要する一切
の費用を織り込んだ上で1品目あたりの単
価を記載すること。
②入札単価については、消費税に係る課税
事業者であるか免税事業者であるかを問わ
ず、見積もった契約金額の110分の100に相
当する金額を記載すること。
(6)その他独立行政法人国立病院機構契約事務
取扱細則(以下、契約細則という。)第22条の
規定に基づき単価契約とする。
2競争参加資格
(1)契約細則第5条の規定に該当しない者であ
ること。なお、未成年者、被保佐人又は被補
助人であっても、契約締結のために必要な同
意を得ているものは、同条中、特別の理由が
ある場合に該当する。
(2)契約細則第6条の規定に該当しない者であ
ること。
(3)厚生労働省競争参加資格(全省庁統一資格)
において「物品の販売」のA、B又はCの等
級に格付され、関東・甲信越地域の競争参加
資格を有する者であること。なお、当該競争
参加資格については、令和8年3月31日付け
号外政府調達第58号の官報の競争参加者の資
格に関する公示の別表に掲げる申請受付窓口
において随時受け付けている。
(4)購入される医薬品を経理責任者が指定する
日時、場所に十分に納品することができるこ
とを証明した者であること。
(5)契約細則第4条の規定に基づき、経理責任
者が定める資格を有する者であること。

参加を希望する者は、封印した入札書に本公
示に示した物品を納入できることを証明する
書類を添付して入札書の受領期限までに提出

類に関し説明を求められた場合はそれに応じ
なければならない。
(4)入札の無効本公告に示した競争参加資格
のない者の提出した入札書、入札者に求めら
れる義務を履行しなかった者の提出した入札
書は無効とする。

(6)落札者の決定方法契約細則第21条の規定
に基づいて作成された予定価格の制限の範囲
内で最低価格をもって有効な入札を行った入
札者を落札者とする。

③have Grade A, B or C in "Sales of prod-
ucts" for participating in tenders by Min-
istry of Health, Labour and Welfare

and agency) in Kanto-Koshinetsu Regi-

④ prove to have obtained license for sell-
ing a medical instrument in accordance
with the pharmaceutical Affairs Law ;
⑤ prove that they can deliver the Phar
maceuticals, on the date and to the place
specified by the Obligating Officer;
6 meet the qualification requirements
which the Obligating Officer may specify
in accordance with Article 4 of the Regu-
lation:
