(6)入札方法入札金額は総価で行う。落札者
の決定は、総合評価落札方式をもって行うの
で、提案に関する性能、機能、技術等(以下
「性能等」という。)に関する書類(以下「総
合評価に関する書類」という。)を提出するこ
と。なお、落札決定に当たっては、入札書に
記載された金額に当該金額の10パーセントに
相当する額を加算した金額(当該金額に1円
未満の端数金額があるときは、その端数金額
を切り捨てるものとする。)をもって落札価格
とするので、入札者は、消費税等に係る課税
事業者であるか免税事業者であるかを問わ
ず、見積もった契約金額の110分の100に相当
する金額を入札書に記載すること。
(7)政府電子調達(GEPS)の利用本案件
は、GEPSで行う。なお、GEPSにより
がたい者は、支出負担行為担当官に書面によ
り申し出た場合に限り紙入札方式によること
ができる。また、電子調達システムの電子入
札機能により入札を行う場合は、個人事業主
に加え、入札参加者から委任等を受けた者の
マイナンバーカードを用いて電子入札を行う
ことができるものとする(入札参加者から委
任等できる者は原則1名とする)。この場合、
事前に所定の手続きにより委任等を受ける者
に対する電子委任状の発行を受けておくこ

2競争参加資格
(1)予算決算及び会計令第70条の規定に該当し
ない者であること。なお、未成年者、被保佐
人又は被補助人であっても、契約締結のため
に必要な同意を得ている者は、同条中、特別
の理由がある場合に該当する。
(2)予算決算及び会計令第71条の規定に該当し
ない者であること。
(3)令和7・8・9年度厚生労働省競争参加資
格(全省庁統一資格)において、厚生労働省
大臣官房会計課長より「役務の提供等」のA
又はBの等級に格付けされ、関東・甲信越地
域の競争参加資格を有する者であること。
(4)厚生労働省から指名停止の措置を受けてい
る期間中の者ではないこと。
(5)資格審査申請書又は添付書類に虚偽の事実
を記載していないと認められる者であるこ

ないと認められる者であること。

当官から当該書類に関し説明を求められた場
合は、これに応じなければならない。
また、入札に参加を希望する者は、上記書
類とあわせて暴力団等に該当しない旨の誓約
書を提出しなければならない。
(4)入札の無効本公告に示した競争参加資格
のない者の提出した入札書、入札者に求めら
れる義務を履行しなかった者の提出した入札
書、その他入札の条件に違反した者の提出し
た入札書は無効とする。
また、入札に参加した者が、(3)の誓約書を
提出せず、又は虚偽の誓約をし、若しくは誓
約書に反することとなったときは、当該者の
入札を無効とするものとする。
