次のとおり一般競争入札に付します。

(6)入札方法落札決定に当たっては、入札書
に記載された金額に当該金額の10パーセント
に相当する額を加算した金額(当該金額に1
円未満の端数があるときは、その端数金額を
切り捨てるものとする。)をもって落札価格と
するので、入札者は、消費税に係る課税事業
者であるか免税事業者であるかを問わず、見
積もった契約金額の110分の100に相当する金
額を入札書に記載すること。
2競争参加資格
(1)独立行政法人国立病院機構契約事務取扱細
則(以下、契約細則という。)第5条の規定に
該当しない者であること。なお、未成年者、
被保佐人又は被補助人であっても、契約締結
のために必要な同意を得ているものは、同条
中、特別の理由がある場合に該当する。
(2)契約細則第6条の規定に該当しない者であ
ること。
(3)厚生労働省競争参加資格(全省庁統一資格)
「役務の提供等」のA、B又はCの等級に格
付され、九州・沖縄地域の競争参加資格を有
する者であること。なお、当該競争参加資格
については、令和7年3月31日付け号外政府
調達第57号の官報の競争参加者の資格に関す
る公示の別表に掲げる申請受付窓口において
随時受け付けている。
(4)契約細則第4条の規定に基づき、経理責任
者が定める資格を有する者であること。
(5)医薬品、医療機器等の品質、有効性及び安
全性の確保等に関する法律に基づいて高度管
理医療機器等販売業の許可を受けていること
を証明した者であること。
(6)医薬品、医療機器等の品質、有効性及び安
全性の確保等に関する法律に基づいて医薬品
の一般販売業の許可を受けていることを証明
した者であること。

参加を希望する者は、封印した入札書に本公
告に示した案件を履行できることを証明する

類に関し説明を求められた場合は、それに応
じなければならない。
(4)入札の無効本公告に示した競争参加資格
のない者の提出した入札書、入札者に求めら
れる義務を履行しなかった者の提出した入札
書は無効とする。

(6)落札者の決定方法本公告に示した案件を
履行できると経理責任者が判断した資料を添
付して入札書を提出した入札者であって、契
約細則第21条の規定に基づいて作成された
予定価格の制限の範囲内で最低価格をもって
有効な入札を行った入札者を落札者とする。
