次のとおり一般競争入札に付します。

(6)入札方法入札金額については、購入物品
のほか、納入に要する一切の費用を含めた額
とすること。なお、落札決定に当たっては
入札書に記載された金額に当該金額の10パー
セントに相当する額を加算した金額(当該金
額に1円未満の端数があるときは、その端数
金額を切り捨てるものとする。)をもって落札
価格とするので、入札者は、消費税及び地方
消費税に係る課税事業者であるか免税事業者
であるかを問わず、見積もった契約金額の
110分の100に相当する金額を入札書に記載す
ること。
2競争参加資格
(1)予算決算及び会計令第70条の規定に該当し
ない者であること。ただし、未成年者、被保
佐人、又は被補助人であって、契約締結のた
めに必要な同意を得ている者については、こ
の限りではない。
(2)予算決算及び会計令第71条の規定に該当し
ない者であること。
(3)厚生労働省競争参加資格(全省庁統一資格)
において「物品の販売」のA、B、C又はD
の等級に格付けされ、近畿地域の競争参加資
格を有する者であること。
(4)購入物品に係る迅速なアフターサービス・
メンテナンスの体制が整備されていることを
証明した者であること。
(5)医薬品医療機器等法に基づく医療機器の販
売業の許可を得ていることを証明した者であ
ること。
(6)その他契約担当役が必要と定める資格を有
する者であること。
