(6)Qualification for participating in the ten-
dering procedures: Suppliers eligible for
participating in the proposed tender are
those who shall:
①not come under Article 5 of the Regula-
tion concerning Contract for National
Hospital Organization (NHO). Further-
more, minors, Person under Conservator-
ship or Person under Assistance that ob-
tained the consent necessary for conclud-
ing a contract may be applicable under
cases of special reasons within the said
clause;
②not come under Article 6 of the Regula-
tion concerning Contract for NHO;
③ have Grade A, D in "Service
Offer" for participating in tenders by Mi-
nistry Health, Labour Welfare
(Single qualification for every ministry
and agency) in Kyusyu-Okinawa Region;
④ meet the qualification requirements
which the Obligating Officer may specify
in accordance with Article 4 of the Regu-
lation :

次のとおり一般競争入札に付します。

(6)入札方法
①(2)で示す品目を区分ごとに取り纏めたも
の(以下「区分」という。)をそれぞれ入札
に付する。
②入札金額については、購入物品のほか、
納入に要する一切の費用を含めた上で区分
ごとにそれぞれの品目の単価を記載するこ

③落札決定については、入札書に記載され
た金額に当該金額の10パーセントに相当す
る額を加算した金額に、当事務所が提示す
る品目ごとの予定数量を乗じて算出した額
の区分ごとの総価(当該金額に1円未満の
端数があるときは、その端数金額を切り捨
てるものとする。)をもって落札価格とする
ので、入札単価については、消費税に係る
課税事業者であるか免税事業者であるかを
問わず、見積もった契約金額の110分の100
に相当する金額を入札書に記載すること。
2競争参加資格
(1)独立行政法人国立病院機構契約事務取扱細
則(以下、契約細則という。)第5条の規定に
該当しない者であること。なお、未成年者、
被保佐人又は被補助人であっても、契約締結
のために必要な同意を得ているものは、同条
中、特別の理由がある場合に該当する。
(2)契約細則第6条の規定に該当しない者であ
ること。
(3)厚生労働省競争参加資格(全省庁統一資格)
「物品の製造」又は「物品の販売」のA、B.
C又はDの等級に格付され、九州・沖縄地域
の競争参加資格を有する者であること。なお、
当該競争参加資格については、令和7年3月
31日付け号外政府調達第57号の官報の競争参
加者の資格に関する公示の別表に掲げる申
請受付窓口において随時受け付ける。
(4)購入物品に係る迅速なアフターサービス・
メンテナンスの体制が整備されていることを
証明した者であること。
(5)契約細則第4条の規定に基づき、経理責任
者が定める資格を有する者であること。

じなければならない。
(4)入札の無効本公告に示した競争参加資格
のない者の提出した入札書、入札者に求めら
れる義務を履行しなかった者の提出した入札
書は無効とする.

(6)落札者の決定方法本公告に示した物品を
納入できると経理責任者が判断した資料を添
付して入札書を提出した入札者であって、契
約細則第21条の規定に基づいて作成された予
定価格の制限の範囲内で最低価格をもって有
効な入札を行った入札者を落札者とする。
