(6)Qualification for participating in the ten-
dering for
participating in the proposed tender are
those who shall:
①not come under Article of the Regula-
tion concerning Contract for National
Hospital Organization (NHO). Further-
more, minors, Person under Conservator-
ship or Person under Assistance that ob-
tained the consent necessary for conclud-
ing a contract maybe applicable under
cases of special reasons within the said
clause:
②not come under Article 6 of the Regula-
tion concerning Contract for NHO;
③ have Grade A, B or C in "Sales of Prod-
ucts", "Service Offer" for participating in
tenders by Ministry of Health, Labor, and
Welfare (Single qualification for every
ministry and agency) in Kinki Region;
④prove to have prepared a system to
provide rapid after-sale service and main-
tenance forthe procured products ;
⑤ meet the qualification requirements
which the Obligating Officer may specify
in accordance with Article 4 of the Regu-
lation :

次のとおり一般競争入札に付します。

(1)品目分類番号4
(2)購入等件名及び数量次の購入物品であっ
て、「使用薬剤の薬価(薬価基準)」に収載され
ているもの。
医薬品一式
品目及び購入予定数量は、入札説明書によ
る。
(3)納入期間令和7年10月1日から令和8年
9月30日まで。
(4)納入場所独立行政法人国立病院機構米子
医療センター
(5)入札方法
①入札金額については、調達物品の本体価
格のほか、納入に要する一切の諸経費を含
め契約金額を見積るものとする。
②落札者決定にあたっては、入札書に記載
された金額に当該金額の10パーセントに相
当する額を加算した金額(当該金額に1円
未満の端数があるときは、その端数金額を
切り捨てるものとする。)をもって評価する
ので、入札者は、消費税に係る課税事業者
であるか免税事業者であるかを問わず、見
積もった契約金額の110分の100に相当する
金額を入札書に記載すること。
(6)その他独立行政法人国立病院機構契約事
務取扱細則(以下、「契約細則」という。)第22
条の規定に基づき単価契約とする。
2競争参加資格
(1)契約細則第5条の規定に該当しない者であ
ること。なお、未成年者、被保佐人又は被補
助人であっても、契約締結のために必要な同
意を得ているものは、同条中、特別の理由が
ある場合に該当する。
(2)契約細則第6条の規定に該当しない者であ
ること。
(3)厚生労働省競争参加資格(全省庁統一資格)
において「物品の販売」のA、B、C又はD
の等級に格付けされ、中国地域の競争参加資
格を有する者であること。なお、当該競争参
加資格については、令和7年3月31日付け号
外政府調達第57号の官報の競争参加者の資格
に関する公示の別表に掲げる申請受付窓口に
おいて随時受け付けている。
(4)医薬品、医療機器等の品質、有効性及び安
全性の確保等に関する法律に基づいて医薬品
の一般販売業者の許可を受けていることを証
明した者であること。
(5)契約細則第4条の規定に基づき、経理責任
者が定める資格を有する者であること。

参加を希望する者は、封印した入札書に本公
告に示した件名を履行できることを証明する
書類を添付して入札書受領期限までに提出し
なければならない。入札者は、開札日の前日
までの間において、経理責任者から当該書類
に関し説明を求められた場合は、それに応じ
なければならない。
(4)入札の無効本公告に示した競争参加資格
のない者の提出した入札書、入札者に求めら
れる義務を履行しなかった者の提出した入札
書は無効とする。

(6)落札者の決定方法契約細則第21条の規定
に基づいて作成された予定価格の制限の範囲
内で最低の価格をもって有効な入札を行った
入札者を落札者とする。

license for selling drugs in accordance
with the Pharmaceutical Affairs Law ;
