あって、「使用薬剤の購入価格」(薬価基準)に

付する。入札金額については、(2)の件名ごと
にそれぞれの単価を記入すること。
なお、落札決定に当たっては、入札書に記
載された金額に当該金額の10パーセントに相
当する額を加算した金額(当該金額に1円末
満の端数があるときは、その端数金額を切り
捨てるものとする。)をもって落札価格とする
ので、入札者は、消費税に係る課税事業者で
あるか免税事業者であるかを問わず、見積
もった契約金額の110分の100に相当する金額
を入札書に記載すること。
(6)その他独立行政法人国立病院機構契約事
務取扱細則(以下、契約細則という。)第22条
の規定に基づき単価契約とする。
2競争参加資格
(1)契約細則第5条の規定に該当しない者であ
ること。なお、未成年者、被保佐人又は被補
助人であっても、契約締結のために必要な同
意を得ているものは、同条中、特別の理由が
ある場合に該当する。
(2)契約細則第6条の規定に該当しない者であ
ること。
(3)厚生労働省競争参加資格(全省庁統一資格)
において「物品の販売」のA、B又はCの等
級に格付され、九州・沖縄地域の競争参加資
格を有する者であること。なお、当該競争参
加資格については、令和7年3月31日付け号
外政府調達第57号の官報の競争参加者の資格
に関する公示の別表に掲げる申請受付窓口に
おいて随時受け付けている。
(4)薬機法に基づいて医薬品の一般販売業の許
可を受けていることを証明した者であるこ
と。
(5)購入される医薬品を経理責任者が指定する
日時、場所に十分に納品することができるこ
とを証明した者であること。
(6)契約細則第4条の規定に基づき、経理責任
者が定める資格を有する者であること。

定める当該医薬品に係わる有効性等に関する
資料及び製剤見本を添付して入札書の受領期
間内までに提出しなければならない。入札者
の作成した資料は経理責任者が技術審査する
ものとし、(5)に定める技術審査基準に照らし
た結果、採用可能と判断された場合のみ当該
入札書を落札決定の対象とする。なお入札者
は、開札日の前日までの間において、経理責
任者から当該医薬品の有効性及び安全性等に
ついて説明を求められた場合はそれに応じな
ければならない。

(5)技術審査基準購入しようとする医薬品の

すものと判断されたものを採用可能とする。
(6)入札の無効本公告に示した競争参加資格
のない者の提出した入札書、入札者に求めら
れる義務を履行しなかった者の提出した入札
書は無効とする。
