次のとおり一般競争入札に付します。

(6)入札方法落札決定は、総合評価の方法を
もって行うので、総合評価のための性能、機
能、技術等に関する書類を提出すること。
入札金額は、購入物品のほか、納入に要す
る一切の費用を含めた額とすること。入札書
に記載された金額に当該金額の10パーセント
に相当する額を加算した金額(当該金額に1
円未満の端数があるときは、その端数金額を
切り捨てるものとする。)をもって評価するの
で、入札者は、消費税に係る課税事業者であ
るか免税事業者であるかを問わず、見積もっ
た契約金額の110分の100に相当する金額を入
札書に記載すること。
2競争参加資格
(1)国立健康危機管理研究機構契約事務取扱細
則(以下、契約細則という。)第6条の規定に
該当しない者であること。なお、未成年者、
被保佐人又は被補助人であっても、契約締結
のために必要な同意を得ているものは、同条
中、特別の理由がある場合に該当する。
(2)契約細則第7条の規定に該当しない者であ
ること。
(3)厚生労働省競争参加資格(全省庁統一資格)
において「物品の製造」及び「物品の販売」
のA、B、C又はDの等級に格付され、関東・
甲信越地域の競争参加資格を有する者である
こと。なお、当該競争参加資格については、
令和7年3月31日付け号外政府調達第57号の
官報の競争参加者の資格に関する公示の別表
に掲げる申請受付窓口において随時受け付け
ている。
(4)購入される物品等を施設が指定する日時、
場所に十分に納品することができることを証
明した者であること。
(5)契約細則第5条の規定に基づき、理事長等
が定める資格を有する者であること。

(3)入札者に要求される事項この一般競争に
参加を希望する者は、封印した入札書に本公
告に示した物品を納入できることを証明する
書類を添付して入札書の受領期限までに提出
しなければならない。入札者は、開札日の前
日までの間において、理事長等から当該書類
に関し説明を求められた場合は、それに応
じなければならない。
(4)入札の無効本公告に示した競争参加資格
のない者の提出した入札書、入札者に求めら
れる義務を履行しなかった者の提出した入札
書は無効とする。
(5)契約書作成の要否要
(6)落札者の決定方法本公告に示した物品を
納入できると総長等が判断した資料を添付し
て入札書を提出した入札者であって、契約細
則第36条の規定に基づいて作成された予定価
格の制限の範囲内で最低価格をもって有効な
入札を行った入札者を落札者とする。

Japan Institute for Health Security;
C have Grade A, B, C or D in "Manufac-
turing of products" or "Seals of products'
for participating in tenders by Ministry
of Health, Labour and Welfare (Single
qualification for every ministry and agen
cy) in Kanto-Koshinetu Region;
D prove to have obtained license for sell-
ing a medical instrument in accordance
with the Pharmaceutical Affairs Law ;
E prove that they can deliver the thing on
the date and to the place specified by the
Obligating Officer;
F meet the qualification requirements
which the Obligating Officer may specify
in accordance with Article 5 of the Regu-
lation
