The Impact of the COVID-19 Pandemic on the Mental Health of Healthcare Workers in China

Abstract
Background: The COVID-19 pandemic has caused significant psychological distress among healthcare workers worldwide. This study aimed to investigate the prevalence and associated factors of mental health problems among Chinese healthcare workers during the early stages of the outbreak.
Methods: A cross-sectional online survey was conducted from February 1 to March 15, 2020. Participants included doctors, nurses, and other medical staff from hospitals across China. Standardized questionnaires were used to assess symptoms of anxiety, depression, insomnia, and post-traumatic stress disorder (PTSD).
Results: Among 1,257 participants, 44.6% reported symptoms of anxiety, 50.7% had depressive symptoms, 36.1% experienced insomnia, and 7.3% met criteria for PTSD. Female gender, working in high-risk departments, and direct contact with confirmed cases were significantly associated with higher levels of psychological distress.
Conclusions: The mental health of healthcare workers in China was severely affected during the initial phase of the COVID-19 pandemic. Urgent interventions are needed to support their psychological well-being and ensure sustainable healthcare delivery.
Introduction
The novel coronavirus disease 2019 (COVID-19), first identified in Wuhan, China, rapidly spread globally, leading to a public health emergency of international concern declared by the World Health Organization on January 30, 2020.
Healthcare workers have been at the forefront of the response, facing unprecedented challenges including long working hours, shortage of personal protective equipment, fear of infection, and separation from family members.
Previous studies have shown that exposure to infectious disease outbreaks can lead to acute stress reactions, burnout, and long-term psychiatric disorders among frontline medical personnel.
However, there is limited data on the specific impact of the COVID-19 pandemic on the mental health of healthcare workers in China, particularly during the critical early period when containment measures were being implemented.
This study seeks to fill this gap by providing empirical evidence on the prevalence and correlates of common mental health issues among Chinese healthcare workers during the initial wave of the pandemic.
