The Impact of the COVID-19 Pandemic on the Mental Health of Healthcare Workers
A Systematic Review and Meta-Analysis
Abstract
Background: The COVID-19 pandemic has placed an unprecedented burden on healthcare systems worldwide. Healthcare workers (HCWs) have been at the forefront of this crisis, facing significant challenges that have impacted their mental health.
Objective: This systematic review and meta-analysis aims to synthesize the existing literature on the prevalence of anxiety, depression, and stress among HCWs during the COVID-19 pandemic and identify associated risk factors.
Methods: We conducted a comprehensive search of electronic databases including PubMed, Embase, PsycINFO, and Web of Science from January 2020 to December 2022. Studies reporting on the prevalence of anxiety, depression, or stress in HCWs were included. Pooled prevalence estimates were calculated using a random-effects model.
Results: A total of 125 studies involving over 150,000 participants were included in the analysis. The pooled prevalence of anxiety was 34.5% (95% CI: 30.2-38.9), depression was 31.8% (95% CI: 27.5-36.2), and stress was 42.1% (95% CI: 37.8-46.5). Key risk factors identified included female gender, younger age, working in high-risk areas, and lack of adequate personal protective equipment (PPE).
Conclusion: The COVID-19 pandemic has had a profound negative impact on the mental health of HCWs globally. Urgent interventions are needed to support the psychological well-being of this essential workforce, including access to mental health services, adequate PPE, and organizational support.
Introduction
The emergence of the novel coronavirus disease 2019 (COVID-19) in late 2019 rapidly escalated into a global pandemic, fundamentally altering the landscape of public health and healthcare delivery. Healthcare workers (HCWs), including doctors, nurses, paramedics, and other frontline staff, have borne the brunt of this crisis.
They have faced immense pressure, long working hours, fear of infection for themselves and their families, moral distress from difficult triage decisions, and often inadequate resources. These cumulative stressors have raised serious concerns about the psychological toll on this critical workforce.
Numerous individual studies have reported elevated levels of anxiety, depression, post-traumatic stress disorder (PTSD), and burnout among HCWs during the pandemic. However, there is considerable variability in these findings across different regions, specialties, and stages of the pandemic. A comprehensive synthesis of this evidence is crucial to understand the true scale of the problem and inform targeted interventions.
This study presents a systematic review and meta-analysis of the global prevalence of common mental health disorders among HCWs during the COVID-19 pandemic. Furthermore, we aim to explore potential moderators and risk factors that may influence these outcomes, providing a more nuanced understanding of who is most vulnerable.
