4電子調達システムの利用
本案件は、電子調達システムで行う。なお、
電子調達システムにより難い者は、支出負担行
為担当官に書面により申し出た場合に限り、紙
入札方式に代えることができる。
5その他
(1)本入札及び契約手続において使用する言語
及び通貨日本語及び日本国通貨、
(2)入札保証金及び契約保証金免除,
(3)入札者に要求される事項この一般競争入
札に参加を希望する者は、令和7年3月28日
午後5時00分までに上記2の競争参加資格を
有することを証明する書類を上記3(1)まで提
出すること。
また、入札に参加を希望する者は、上記確
認書類と併せて暴力団等に該当しない旨の誓
約書を提出しなければならない。
入札者は、支出負担行為担当官から当該書
類に関し説明を求められたときは、それに応
じなければならない。
(4)郵送による入札郵送(書留郵便に限る。)
による提出の場合は、上記3(1)宛てに入札書
の受領期限までに到着するよう送付しなけれ
ばならない。なお、未着の場合、その責任は
参加者に属するものとする。郵送の場合、担
当者の職氏名及び連絡先を明記すること。
(5)入札の無効本公告に示した競争参加資格
のない者の提出した入札書、入札者に求めら
れる義務を履行しなかった者の提出した入札
書、その他入札の条件に違反した者の入札書
は無効とする。
(6)契約書作成の要否要。
(7)落札者の決定方法予決令第79条の規定に
基づいて作成された予定価格の制限の範囲内
で最低価格をもって有効な入札を行った入札
者を落札者とする。

1競争入札に付する事項

(5)入札方法入札金額は総価で行う。落札者
の決定に当たっては、総合評価落札方式を
もって行うので、提案に係る性能、機能、技
術等に関する書類を提出すること。なお、落
札決定に当たっては、入札書に記載された金
額に当該金額の10%に相当する額を加算した
金額(当該金額に1円未満の端数があるとき
は、その端数金額を切り捨てるものとする。)
をもって落札価格とするので、入札者は、消
費税に係る課税事業者であるか免税事業者で
あるかを問わず、見積もった契約金額の110
分の100に相当する金額を入札書に記載し入
札すること。
2競争参加資格
(1)予算決算及び会計令(以下「予決令」とい
う。)第70条及び第71条の規定に該当しない者
であること。なお、未成年者、被保佐人又は
被補助人であっても、契約締結のために必要
な同意を得ている者は除くものとする。
(2)令和04・05・06年度厚生労働省競争参加資
格(全省庁統一資格)「役務の提供等」で「A」
等級に格付けされている者であること。
(3)資格審査申請書に虚偽の事実を記載してい
ないと認められる者であること。
(4)経営の状況又は信用度が極度に悪化してい
ないと認められる者であること。
(5)労働保険及び厚生年金保険・全国健康保険
協会管掌健康保険・船員保険又は国民年金の
未適用及びこれらに係る保険料の滞納がない
こと(入札書提出期限の直近2年間の保険料
の滞納がないこと。)
(6)厚生労働省から指名停止の措置を受けてい
る者でないこと。
(7)その他予決令第73条の規定に基づき、支出
負担行為担当官が定める資格を有する者であ
ること。
