次のとおり一般競争入札に付します。

(2) 調達件名及び予定数量 全国健康保険協会大阪支部移転に伴う新規什器購入設置業務委託一式

(6) 入札方法 総価にて入札に付する。落札決定に当たっては、入札書に記載された金額をもって落札判定を行うので、入札者は、消費税等に係る課税事業者であるか免税事業者であるかを問わず税抜額を入札書に記載すること。
2 競争参加資格
(1) 全国健康保険協会会計細則第30条及び第31条の規定に該当しない者であること。
(2) 令和4・5・6年度厚生労働省競争参加資格（全省庁統一参加資格）「物品の販売」及び「役務の提供等」のA、B又はCの等級に格付けされ、近畿地域の競争参加資格を有するものであること。なお、当該競争参加資格については、令和6年3月29日付け号外政府調達第58号の官報の競争参加者の資格に関する公示の別表に掲げる申請受付窓口において随時受け付けている。
(3) 資格審査申請書又は添付書類に虚偽の事実を記載していないと認められる者であること。

4 その他
(1) 契約手続において使用する言語及び通貨
日本語及び日本国通貨に限る。
(2) 入札保証金及び契約保証金 全額免除とす
る。
(3) 入札者に要求される事項 この一般競争に
参加を希望する者は、競争参加資格に関する
証明書等を令和6年10月8日11時00分までに
提出しなければならない。入札者は、開札日
の前日までの間において、全国健康保険協会
事務担当者から当該書類等に関し説明を求め
られた場合は、それに応じなければならない。
(4) 入札の無効 本公告に示した競争参加資格
のない者の提出した入札書、入札者に求めら
れる義務を履行しなかった者の提出した入札
書、その他入札条件に違反した入札は無効と
する。
(5) 契約書作成の要否 要
(6) 落札者の決定方法 本公告に示した業務を
実施できると全国健康保険協会大阪支部長が
判断した資料を添付して入札書を提出した入
札者であって、全国健康保険協会会計規程第
32条の規定に基づいて作成された予定価格の
制限の範囲内で、最低価格をもって有効な入
札を行った入札者を落札者とする。

5 Summary

(6) Qualifications for participating in the
tendering procedures: Suppliers eligible for
participating in the proposed tender are
those who shall :
① Not come under the Article 30 of the
Japan Health Insurance Association Or-
der concerning the accounts regulations
② Not come under the Article 31 of the
Japan Health Insurance Association Or-
der concerning the accounts regulations
③ Possess the Grade “A”, “B” or “C” in
“Sales of product” and “Offer of services
etc.” in the Kinki Area in terms of the
qualification for participating in tenders
by the Ministry of Health, Labour and
Welfare (Single qualification for every
ministry and agency) in fiscal year 2022,
2023, and 2024
④ Prove to have no false statement in
tendering application forms or attached
documents
⑤ Prove neither the business condition
nor credibility is deteriorating
⑥ Prove to have the ability to provide
sufficient service with certainty
⑦ Being the person who does not take
compensation for damages request from
Japan Health Insurance Association
⑧ Do not come out of Japan Health Insur-
ance Association during a period taking a
nomination stop about duties
