1 調達内容

(2) 調達件名及び数量 令和6年度国立県営福岡障害者職業能力開発校訓練用機器一式の調達（リース）
(3) 調達件名の仕様等 入札説明書及び仕様書による。
(4) 履行期間 納入日から令和11年3月31日まで

(6) 入札方法 入札書に記載された金額に課税対象金額の10%に相当する額を加算した金額（当該金額に1円未満の端数があるときは、その端数金額を切り捨てるものとする。）をもって落札価格とするので、入札者は、消費税等に係る課税事業者であるか免税事業者であるかを問わず、見積もった契約金額の110分の100に相当する金額を入札書に記載すること。
2 競争参加資格
(1) 令和04・05・06年度厚生労働省競争参加資格（全省庁統一資格）において、九州・沖縄地域で「役務の提供等」の「B」「C」又は「D」等級に格付けされているもの。
(2) 予算決算及び会計令第70条及び第71条の規定に該当しない者。
(3) 厚生労働省から指名停止の措置を受けている期間中ではない者。
(4) 経営状態が著しく不健全であると認められる者ではないこと。
(5) 商法その他の法令の規定に違反した営業を行った者ではないこと。
(6) 資格審査申請書及びその添付書類の重要な事項又は事実について虚偽の記載をし、又は記載をしなかった者ではないこと。
(7) 社会保険等（厚生年金保険、健康保険（全国健康保険協会が管掌するもの）、船員保険、国民年金、労働者災害補償保険及び雇用保険をいう。）に加入し、該当する制度の保険料の滞納がない者。（加入義務がないものを除く。）
(8) その他予決令第73条の規定に基づき、支出負担行為担当官が定める資格を有する者であること。
3 入札書の提出場所等

4 電子調達システムの利用 本件は、電子調達システムを利用した応札及び入札手続きにより実施するものとする。ただし、紙による入札書の提出も可とする。詳細については、入札説明書のとおり。

6 Summary

(3) Nature and quantity of the products to be purchased: Training equipment lease (Fukuoka Vocational Training School for the Disabled), 1 set.
(4) Lease period: From delivery date through 31 March 2029
(5) Fulfillment place: described in the specification
(6) Qualification for Participating in the tendering procedures: Suppliers eligible for participating in the proposed tender are those who shall;
(1) Have Grade “B”, “C” or “D” on “offer of services etc.” in Kyushu-Okinawa Region in terms of qualification for participating in tenders by Ministry of Health, Labour and Welfare (Single qualification for every ministry and agency) in the fiscal years, 2022, 2023 and 2024;
(2) Not come under Article 70 and 71 of the Cabinet Order concerning the Budget, Auditing and Accounting;
(3) Not be currently under a nomination stop as instructed by the Ministry of Health, Labour and Welfare;
(4) Prove neither the business condition nor credibility is deteriorating;
(5) Do not perform business that violates the provisions of the Commercial Code and other laws and regulations;
(6) Prove to have no false statement in tendering documents;
(7) Participate in social insurance, etc. (Welfare annuity insurance, Health insurance (the thing which association of national health insurance has jurisdiction over), Seamen’s insurance, national pension, industrial Accident Compensation insurance, employment insurance), and there is no delinquency in insurance premiums. (However, excluding those who are not obligated to join);
(8) A person who has the qualifications determined by the officer in charge of spending activities pursuant to the provisions of Article 73 of the Cabinet Order concerning the Budget, Auditing and Accounting;
