なお、落札決定に当たっては、入札書に記載された金額に当該金額の10パーセントに相当する額を加算した金額（当該金額に1円未満の端数があるときは、その端数金額を切り捨てるものとする。）をもって落札価格とするので、入札者は、消費税に係る課税事業者であるか免税事業者であるかを問わず、見積もった契約金額の110分の100に相当する金額を入札書に記載すること。
2 競争参加に必要な資格
(1) 予算決算及び会計令（昭和22年勅令第165号。以下「予決令」という。）第70条の規定に該当しないこと。なお、未成年者、保佐人又は被保佐人であって、契約締結のために必要な同意を得ている者は、同条中、特別の理由がある場合に該当する。
(2) 予決令第71条の規定に該当しない者であること。
(3) 令和04・05・06年度厚生労働省競争参加資格（全省庁統一資格）の「役務の提供等」において「A」「B」又は「C」の等級に格付けされており、営業品目「賃貸借」を有する者で、競争参加地域が「関東・甲信越」の資格を有する者であること。
(4) 厚生労働省から指名停止の措置を受けている期間中の者でないこと。
(5) 資格審査申請書又は添付書類に虚偽の事実を記載していないと認められる者であること。
(6) 社会保険等（厚生年金保険、健康保険（全国健康保険協会が管掌するもの）、船員保険、国民年金、労働者災害補償保険及び雇用保険をいう。）に加入し、該当する制度の保険料の滞納がないこと。（直近2年間の社会保険等の未納がないこと。）
(7) 経営の状況又は信用度が極度に悪化していないと認められる者であること。
(8) 厚生労働省関係法令（働き方改革関係法令含む）を遵守していること。
(9) 会社更生法に基づく更生手続開始の申立て及び民事再生法に基づく再生手続開始の申し立てがなされていない者であること。
(10) この入札の入札書提出期限の直近1年間において、厚生労働省が所管する法令に違反したことにより送検され、行政処分を受け、又は行政指導（行政機関から公表されたものに限る。）を受けた者にあっては、本件業務の公正な実施又は本件業務に対する国民の信頼の確保に支障を及ぼすおそれがないこと。

(iv) Bidding Method Enter the total price as
the bid amount. In addition, when determin-
ing the successful bid, the successful bid
price will be the amount stated in the bid
document plus 10% of said amount (if the
amount has a fraction of less than 1 yen, the
fraction will be rounded down). Therefore,
regardless of whether the bidder is a tax-
able business entity or a tax-exempt busi-
ness entity, the bidder must state in the bid
document an amount equivalent to 110/
100 of the estimated contract amount.
(v) Qualifications required to participate in
the competition
① Not falling under the provisions of Ar-
ticle 70 of the Budget, Settlement and Ac-
counting Order (Imperial Ordinance No.
165 of 1947; hereinafter referred to as the
“Budget and Settlement Order”). In addi-
tion, minors, curators, or persons under
curatorship who have obtained the con-
sent necessary for entering into a con-
tract fall under the special circumstances
of the same article.
② Not falling under the provisions of Ar-
ticle 71 of the Budget and Settlement Or-
der.
③ The applicant must be rated as “A,”
“B,” or “C” in the “Provision of Services,
etc.” category of the Ministry of Health,
Labor, and Welfare’s qualification to par-
ticipate in competitive bidding (unified
qualification for all ministries and agen-
cies) for fiscal years 2022, 2023, and 2024,
and must have the business item
“Lease” and be qualified for the “Kanto/
Koshinetsu” region of the bidding.
④ The applicant must not be currently
subject to a suspension of bidding by the
Ministry of Health, Labor, and Welfare.
⑤ The applicant must be deemed not to
have stated any false facts in the qualifi-
cation examination application form or
attached documents.
⑥ The applicant must be enrolled in so-
cial insurance, etc. (meaning employees’
pension insurance, health insurance (ad-
ministered by the National Health Insur-
ance Association), seamen’s insurance,
national pension, workers’ accident com-
pensation insurance, and employment in-
surance) and must not be in arrears with
insurance premiums for the relevant sys-
tem. (No unpaid social insurance, etc. for
the past two years.)
⑦ The applicant must be deemed not to
have experienced an extreme deteriora-
tion in the business situation or cre-
ditworthiness.
