(6) 入札方法 落札決定に当たっては、入札書 に記載された金額に当該金額の100分の10に 相当する額を加算した金額（当該金額に1円 未満の端数があるときは、その端数金額を四 捨五入した金額）をもって落札価格とするの で、入札者は、消費税及び地方消費税に係る 課税事業者であるか免税事業者であるかを問 わず、見積もった契約希望金額の110分の100 に相当する金額を入札書に記載すること。
2 競争参加資格
(1) 中日本高速道路株式会社契約規則（中日本 高速道路株式会社規程第25号）第11条の規定 に該当しない者であること。
(2) 次に定める書類及び(3)(4)に定める証明書 （以下「証明書等」という）を期限までに提 出し、一般競争入札参加資格の確認を受けた 者であること。
① 商業登記簿謄本の写し
② 会社の経歴を記載した書類
③ 直近の営業報告書及び財務諸表
④ 過去3ヶ月以内に発行された納税証明書 （国税通則法施行規則）別紙第9号様式（そ の3、その3の2、その3の3のいずれか） の写し
(3) 塩化ナトリウムについて、1年間（12ヶ月 間）で800t以上の販売実績、製造実績又は 納入実績のいずれかがあることを証明した者 であること。
(4) 納入指図をした時刻から72時間以内に下記 条件を満たすことを証明した者であること。 ・当購入の仕様書に規定する凍結防止剤を八 王子、大月、甲府保全・サービスセンター 管内のそれぞれの剤倉庫へ納入できるこ と。
・凍結防止剤の指図1回当たりの最大納入数 量として、八王子保全・サービスセンター 管内へ91t、大月保全・サービスセンター 管内へ134t、甲府保全・サービスセンター 管内へ104tをそれぞれ納入できること。 納入計画書に以下の内容を記載して提出す ること。
① 凍結防止剤の原産地
② 在庫管理する倉庫の容量（凍結防止剤の t数）、所在地、位置図
③ 在庫管理する倉庫から八王子、大月、甲 府の各保全・サービスセンター管内それぞ れの剤倉庫までの運搬経路、運搬時間、運 搬車両等の車種（積載可能t数）及び台数。 ただし、運搬時間を算出する為に用いる走 行速度は、一般道30km/h、高速道路及び自 動車専用道路70km/hとする。
(5) 入札書の提出期限から開札の日までの期間 に資格登録停止又は取引停止を受けている期 間中でないこと。

(3) 入札者に要求される事項 この一般競争に参加を希望する者は、本公告に示した物品を納入できることを証明する証明書等を期限までに提出しなければならない。入札者は、開札日の前日までの間において、契約責任者から当該書類に関し説明を求められた場合は、それに応じなければならない。
(4) 入札の無効 本公告に示した競争参加資格を有しない者がした入札及び入札に関する条件に違反した入札は無効とする。
(5) 契約書の作成の要否 要。
(6) 落札者の決定方法 本公告に示した物品を納入できると契約責任者が判断した資料を提出した入札者であって、契約制限価格の範囲内で最低価格をもって有効な入札を行った入札者を落札者とする。

③ Show a proof of the actual selling, manufacturing or delivering of 800 ton/year of sodium chloride used as de-icing agents for roads on the contract
④ Have proven to be able to bring the requested Sodium Chloride to the following places, respectively, within 72 hours after orders are issued
• The Sodium Chloride Warehouses of Hachioji Maintenance/Customer Service Center : total 91 ton
• The Sodium Chloride Warehouses of Otsuki Maintenance/Customer Service Center : total 134 ton
• The Sodium Chloride Warehouses of Kofu Maintenance/Customer Service Center : total 104 ton
⑤ Submit the Supply Planning Report which includes the following contents
• The place of production of the sodium chloride country
• The capacity of the stockyard, the address and Location map
• The delivering route, time and the number and the type of vehicle

2 競争参加資格
(1) 中日本高速道路株式会社契約規則（中日本高速道路株式会社規程第25号）第11条の規定に該当しない者であること。
(2) 次に定める書類及び(3)(4)に定める証明書（以下「証明書等」という）を期限までに提出し、一般競争入札参加資格の確認を受けた者であること。

(3) 塩化ナトリウムについて、1年間（12ヶ月間）で1,200t以上の販売実績、製造実績又は納入実績のいずれかがあることを証明した者であること。
(4) 松本保全・サービスセンターから半径100km以内に位置する400t以上の備蓄倉庫を保有もしくは借用し、納入指図を通知した時刻から24時間以内に各剤倉庫に総計170tを搬入できることを証明した者であること。 納入計画書に以下の内容を記載して提出すること。
① 凍結防止剤の原産地
② 在庫管理する倉庫の容量（凍結防止剤のt数）、所在地、位置図
③ 在庫管理する倉庫から松本保全・サービスセンター管内それぞれの剤倉庫までの運搬経路、運搬時間、運搬車両等の車種（積載可能t数）及び台数。
ただし、運搬時間を算出する為に用いる走行速度は、一般道30km/h、高速道路及び自動車専用道路70km/hとする。
(5) 入札書の提出期限から開札の日までの期間に登録停止又は取引停止を受けている期間中でないこと。
