(5) 調達に必要とされる基本的な要求要件
(1) 診療系・看護系・医事系の3要素を対等
な関係において統合し、本学附属病院の診
療業務を総合的に支援しつつ、病院業務を
円滑に運営・監理できるとともに、全体の
セキュリティ管理機能を備えること。
(2) 病院運営監理機能として、システム保守
管理、ユーザ管理、アクセス権限及び履歴
管理、人事管理、物流管理、病床管理のほ
か医療従事者を対象とした広報などのメッ
セージング機能、リスクマネジメントの
サポート機能等を備えること。
(3) 本調達に含まれるサブシステムならびに
本院既存の各サブシステムと相互に有機的
に連携し、診療情報の開示や診療情報連携、
リスク要因の解析、外部評価などに耐えら
れる粒度及び可用性を備えたデータの蓄
積・抽出・再利用を前提とした電子化診療
録を構成すること。
(4) 診療に関わるオーダリング機能の対象と
して、患者基本情報、予約(外来、入院、
他科・他部署受診等)、検査(検体・生理・
放射線・病理等)、処方(院内・院外等)、
注射(外来・入院等、レジメン、院内調剤
等)、処置(外来、病棟、放射線治療、内
視鏡、透析、理学療法等)、輸血、手術、
歯科処置、看護ケア、患者移動(入退院を
含む転棟・転科・主治医の変更・転床・外
泊等)、食事、ICDコードに基づく病名、
治験等を含むこと。これらのオーダはタイ
ムスタンプ・実施者・実施場所等の属性を
保持した実施加力に基づき運用を前提と
し、時系列に沿った一覧機能を有するとと
もに、変更履歴を備えていること。
(5) 電子化診療録機能及び診療にかかわる
オーダリング機能を用いた、クリニカルパ
スとしての運用が可能であること。加えて、
インシデント記録・蓄積・解析機能を備え
ること。
(6) 医事会計サブシステムは、電子レセプト
生成及び経営分析に必要となるオーダ情報
の全ての項目を取り込んで蓄積し、入力の
自動化に対応すること。
(7) 要求システムの各端末は、マルチウィン
ドウやGUIを採用し、操作性の統一を図
り、快適な作業環境を提供すること。また、
無線LANなど不安定なネットワーク接続
環境下でも継続的に利用できること。さら
に、各端末の一元管理を実現するために統
合的な機能を提供すること。
(8) 情報伝達の接続手順はTCP/IP、画
像はDICOM ver.3.0以降・JPEG
2000、電文情報はHL7 ver.2.4以降とX
MLの利用を前提とし、マルチベンダー
マルチプラットホームに対応可能なオープ
ンシステムであること。
(9) 要求システムは、現有システムに接続し
運用している各部門のサブシステム及び医
療機器等とEthernetにより構築されている
院内ネットワークを介しTCP/IPを基
本として接続できること。各サブシステム
間通信、及びセキュリティ監査のために必
要となる情報を提供すること。
(10) 要求システムは、いわゆるノンストップ
システムとして構成され、且つ迅速なレス
ポンスを確保できる性能を有すること。
(11) 要求システム全体の保守管理体制及び操
作教育体制を有し、これを行うこと。
(12) 現有病院情報管理システムに蓄積された
診療情報等の必要となるデータ群及びマス
ター等を移行し利用可能とすること。
(13) 基本パッケージにて実現困難なアプリ
ケーションを機動的に開発可能なプログラ
ミング環境を装備し、要求システム上で運
用上必要となる技術情報等を提供するこ
と。
(14) 今後、要求システムからのデータ移行が
必要となる場合(契約期間終了時のデータ
移行を含む)には、本学に対し、そのため
に必要となる情報、ならびに汎用的なデー
タ形式で全データを提出する作業を行うこ
と。また、本学及び本学が認めた第三者が
これらを移行作業に利用することを認める
こと。
2 資料及びコメントの提供方法 上記1(2)の物
品に関する一般的な参考資料及び同(5)の要求要
件等に関するコメント並びに提供可能なライブ
ラリーに関する資料等の提供を招請する。

5 その他 この導入計画の詳細は導入説明書に
よる。なお、本公表内容は予定であり、変更す
ることがあり得る。
6 Summary
(1) Classification of the products to be pro-
cured: 14
(2) Nature and quantity of the products to be
rent: Integrated Hospital Information Sys-
tem 1 Set

(4) Basic requirements of the procurement :
(1) Integrate the three elements of medical
care, nursing, and medical administration
on an equal footing, while comprehen-
sively supporting the clinical operations
of our university hospital, enabling
smooth operation and management of
hospital activities, and providing overall
security management functions.
(2) Provide functions for operating and
managing the hospital, including system
maintenance and management, user man-
agement, access rights and history man-
agement, personnel management, logist-
ics management, bed management, mes-
saging functions for public relations tar-
geted at healthcare workers, and risk ma-
nagement support functions.
(3) Ensure that the subsystems included in
this procurement, as well as the existing
subsystems of the hospital, are organical-
ly integrated with each other to form an
electronic medical record that supports
the accumulation, extraction, and reuse
of data with the granularity and avai-
lability capable of withstanding the dis-
closure of clinical information, clinical
information linkage, risk factor analysis,
and external evaluations.
(4) The ordering functions related to clin-
ical care should include patient basic in-
formation, appointments (outpatient, in-
patient, referrals to other departments or
sections, etc.), tests (specimens, physiol-
ogy, radiology, pathology, etc.), prescrip-
tions (in-house, out-of-house, etc.), in-
jections (outpatient, inpatient, regimens,
in-house dispensation, etc.), treatments
(outpatient, wards, radiation therapy, endoscopy, dialysis, physiotherapy, etc.), blood transfusions, surgeries, dental treatments, nursing care, patient transfers (including admissions and discharges, transfers between wards or departments, changes in attending physician, bed transfers, temporary leaves, etc.), meals, disease names based on ICD codes, clinical trials, etc. These orders should be operated based on execution inputs that retain attributes such as timestamps, executors, and execution locations, have a chronological list function, and include change histories.
(5) Enable operation as a clinical pathway using electronic medical record functions and ordering functions related to clinical care. Additionally, provide incident recording, accumulation, and analysis functions.
(6) Provide a medical accounting subsystem that incorporates and stores all order information items necessary for generating electronic receipts and management analysis, and supports automated input.
(7) Each device of the required system should adopt multi-window and GUI, ensuring unified operability and providing users with a comfortable working environment. Additionally, the devices should be continuously usable even in unstable network connection environments such as wireless LAN. Furthermore, provide integrated functions that enable unified management of each device.
(8) The connection procedures for information transmission should be an open system that supports multi-vendor, multi-platform compatibility, based on the use of TCP/IP for communication, DICOM ver. 3.0 or later and JPEG2000 for images, and HL7 ver. 2.4 or later and XML for message information.
(9) The required system should be able to connect via Ethernet through the hospital's internal network, which is built on TCP/IP, with the subsystems and medical devices of each department currently connected and operated in the existing system. It should provide the necessary information for communication between subsystems and for security audits.
(10) The required system should be configured as a non-stop system, with performance that ensures rapid response times.
(11) Have an organization capable of maintenance management and operational training for the entire required system, and implement them.
(12) Transfer and use the necessary data groups and masters accumulated in the existing Integrated Hospital Information System.
(13) Equip it with a programming environment that allows for agile development of applications that are difficult to realize with the basic package and provide the necessary technical information for operating on the required system.
(14) After the system is operational, when data migration is necessary from the required system or when data migration is required at the end of the contract period, provide our university with the necessary information and supply all data in a generic data format. Additionally, allow our university and any third party recognized by our university to utilize all provided data.
